Position Overview The Client Services & Community Nurse strengthens trust, visibility, and connection between Senior Helpers and the wider community while also supporting our internal care operations. This hybrid role blends clinical insight, relationshipbuilding, client care, and care coordination. The ideal candidate is an LPN or RN who is warm, confident, and comfortable engaging with families, seniors, community partners, and our internal care team. This role includes occasional handson client care, especially during new client on-boarding, complex situations, or when additional clinical oversight is needed. Key Responsibilities Client Care & Support (HandsOn as Needed) Provide direct client care within LPN/RN scope during on-boarding, reassessments, or when client needs change. Observe and report changes in condition, ensuring timely communication with families and the office team. Care Planning & Coordination Conduct inhome assessments to understand client needs, risks, preferences, and goals. Develop and update individualized care plans in collaboration with the Client Services team. Provide clinical insight to guide caregiver matching, scheduling decisions, and service adjustments. Participate in case reviews and help ensure care plans remain accurate, safe, and personcentered. Support quality assurance by performing periodic checkins and followup visits. Client & Family Relations Serve as a trusted, compassionate point of contact for families navigating senior care decisions. Offer clear explanations of care options, safety considerations, and service pathways. Support families during transitions such as hospital discharge, cognitive decline, or increased care needs. Help resolve concerns with empathy, professionalism, and a solutionsfocused approach. Community Engagement & Outreach Build and maintain relationships with hospitals, clinics, senior centres, retirement communities, and local organizations. Represent Senior Helpers at community events, health fairs, and educational sessions. Deliver short presentations on aging, safety, dementia awareness, and inhome care. Act as a warm, credible ambassador for Senior Helpers in the Okanagan. Internal Collaboration Work closely with the Client Services team to ensure smooth coordination of care. Provide clinical insight to support caregiver training, safety practices, and service quality. Participate in team meetings and contribute to continuous improvement initiatives. Support the development of educational materials and community resources. Qualifications LPN or RN designation (current or retired and in good standing). Strong communication and interpersonal skills. Comfort with public speaking and community engagement. Experience working with seniors, dementia, or inhome care is an asset.
