Duties and Services Individualized pharmacotherapy Counselling patients about their medication therapy Work in collaboration with physicians and other healthcare providers to develop, implement and monitor therapeutic plans Assist in the development of drugtherapy guidelines for individual disease conditions Prepare and submit case studies, articles and research for publication in professional journals Act as a department resource person within your assigned area Participate in medicationuse evaluations, qualityimprovement initiatives and clinical drug trials Provide mentorship and supervision to pharmacy students, residents and new staff Ensure prescriptions are validated, dispensed accurately and that medication safety concerns are resolved expediently; document all professional practice activities proficiently Qualifications Current registration as a Pharmacist with the College of Pharmacists of British Columbia. Detailed Overview Situated within identified Correctional Health Services locales, this position provides clinical pharmacy services to clients of the Correctional Health Services. As a member of an interprofessional team, you will work directly with clients to identify and resolve actual and potential drugtherapy problems through comprehensive medication management. Responsibilities Identify and resolve actual and potential drugtherapy problems in collaboration with the client and other members of the healthcare team. Provide comprehensive medication management and pharmaceutical care, monitoring medical conditions and ongoing drug therapy for effectiveness, adherence, adverse reactions, drug interactions and both therapeutic and laboratory incompatibilities; communicate findings, recommendations and plans to clients and appropriate healthcare practitioners. Consult with clients and the healthcare team to develop clientspecific therapeutic plans based on chart review, client information and assessment of clinical responses. Maintain appropriate documentation in the health record in accordance with organizational standards and practices. Use appropriate resources to support professional practice and client care, including provincial programs such as academic detailing, optimal druguse initiatives, opioid stewardship and deprescribing. Establish linkages, as required, with health authority, community pharmacy services and other relevant community services to support continuity of care for clients. Provide drug information, consultation and other support services to medical, nursing, pharmacy and other healthcare team members by communicating concerning drugrelated issues, questions and clientspecific therapeutic regimens. Conduct client counselling and education regarding drug therapy. Provide inservice education to the healthcare team where appropriate. Provide instruction and supervision to students such as University Pharmacy Undergraduates and Pharmacy Residents. Participate in various committees as appointed, to provide guidance and advice on drugtherapy matters. Participate in qualityimprovement activities through reviewing current practice, identifying actual or potential problems and recommending and evaluating modifications. Maintain workload records and metrics as required. Participate in programs such as Patient Safety and DrugReaction Reporting by collecting and compiling relevant data as applicable. Share the responsibility for orientation of new personnel and act as a role model. Maintain a working knowledge of PharmaCare policies and procedures. Perform other related duties as assigned. Education and Experience Current registration as a pharmacist with the College of Pharmacists of British Columbia. Two (2) years recent, relevant patientcare experience or an equivalent combination of education, training and experience. Valid BC Drivers Licence and access to a personal vehicle for businessrelated purposes. Skills and Abilities Broad knowledge of organizational policies, procedures and standards of care. Knowledge of evidencebased clinical pharmacy practice. Knowledge of medicationuse evaluation. Knowledge of research processes and methodology. Knowledge of drug distribution processes in a healthsystem setting. Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision. Ability to communicate effectively both verbally and in writing. Ability to establish and maintain effective working relationships with other health practitioners, clients, family members and the public. Ability to organize and prioritize work. Ability to operate related equipment. Physical ability to carry out the duties of the position.
