Community Social Worker - Grief and Bereavement position focused on supporting clients in North Brampton from now to March 31, 2027. Temporary, part-time to March 31, 2027.Compensation: $36.00-$45.00/hourHours of Work: This a part-time, .5 Full-time equivalent position working with a job-share partner who fulfills the other half of the role. There is flexibility; however, it could be three 8-hour days one week and two 8-hour days the following. We live our core values that show our caring and form the basis of all our decisions and actions including Dignity, Courage, Accountability, Collaboration, Innovation, DEI (Diversity, Equity & Inclusion).Position SummaryThe Supportive Care Program at Bethell Hospice is available to all clients, caregivers and families (of all ages) in the Dufferin/Caledon area both before and following a clients death. This role provides social work, grief and bereavement support in North Brampton in a variety of ways to meet the individual and unique needs of each client.The Community Social Worker - Grief and Bereavement is focused on the psychological, emotional, spiritual and mental well-being of persons who have experienced a death, loss and/or those who are experiencing an anticipatory loss, and their partners and family members, as they deal with issues of self-esteem, identity, adaptation to illness and its consequences, social functioning and relationships, communication and spiritual suffering.Location: - this is a hybrid position. Most of the work is community-based in North Brampton and care will be provided based on client needs and their location (home, long-term care, hospital, in the Bethell-south office in Inglewood), with the ability to also work remotely from home. When working in the community, travel and mileage expenses will be paid in accordance with Bethell Hospices Business Travel policy.Education/Qualifications Masters Degree in Social Work is required. Please note that this funding is specific to this qualification so we are unable to consider any other academic backgrounds Minimum of three years of recent work experience as a Social Worker, preferably in the palliative care/grief space Current registration with the Ontario College of Social Workers and Social Service Workers as a Social WorkerAbilities/Requirements Must have a car, valid drivers license and appropriate insurance. Must provide proof of annual professional membership. Self-starter who thrives in an environment where s/he can be equally successful working independently as well as collaboratively in a team environment. Experienced in working successfully with a diverse client group Knowledge of and experience working with client databases, e.g. InfoAnywhere. Expertise in providing palliative/bereavement counselling services. Able to work effectively with community agencies to secure resources for clients. Experience providing services for clients in community settings. Fluency in languages prevalent in the Region in addition to English.Key Responsibilities Provides individual client counselling (in home, in hospice office, by telephone, or virtually through Microsoft Teams) Leads Group support and educational workshops (internally and externally) Creates Monthly mailing of bereavement support educational materials Acts as a Referral source for other community resources; primary resource and consultant for partner agencies, team members and community on issues related to bereavement and spiritual care. Maintains comprehensive data and a knowledge base of community services and resources. Implements outcome measurement strategies and meets targets and standards for the program. Submits monthly reports and statistical data to their Manager. Participates in sessions to promote awareness within the community of Bethell Hospice services Works collaboratively with all the local hospices, Ontario Health at Home, hospitals, community health care agencies, physicians, other spiritual care providers, and volunteers.Clinical portfolio including two primary client groups 1) those with palliative care needs and their caregivers, and 2) the bereaved (including some pediatric and youth clients).Attributes/Skills The Community Social Worker has attributes of honesty, integrity, trustworthiness, compassion, understanding, patience, self-awareness, and excellent ethical standards.They will practice with a high level of autonomy and will also require flexibility to be able to work with clients/families and the inter-professional team.They are client-focused, accessible, inspire trust and are respectful.Excellent organizational ability, critical thinking and decision-making skills, and a commitment to inclusion, equity and diversity.General Use of personal vehicle for work related travel as required.
