About the Role As a Nurse Team Leader, you will work across four complex care packages, delivering direct care and providing daytoday clinical leadership. You will support the Clinical Lead with competency signoff, supervision, training, and quality assurance. 1. Clinical Care Delivery - Deliver direct nursing care in line with individual clinical needs - Carry out clinical interventions, monitoring and escalation - Ensure safe, personcentred care at all times - Act as a visible, supportive clinical leader - Coach, mentor, and supervise Support Workers (HCAs) - Support induction and training of new staff - Provide both formal and informal supervision with the Clinical Lead 3. Competency & Workforce Assurance - Support competency assessments and signoff - Ensure staff are safe and competent before lone working - Identify training needs and elevate appropriately - Maintain accurate and auditable competency records 4. Clinical Governance & Quality - Ensure care aligns with care plans, risk assessments, and policies - Keep highquality clinical documentation and daily notes - Identify risks early and take corrective action - Escalate safeguarding and incidents appropriately - Support audits, reviews, and ongoing service improvements 5. MultiDisciplinary & Stakeholder Working - Liaise professionally with families, Case Managers, ICB and MDT colleagues - Provide structured updates to the Clinical Lead and wider care team - Support implementation of MDT treatment plans 6. Operational Support - Work closely with Care Coordination to maintain rota stability - Assist during periods of risk, transition, or package change - Support cover plans where clinically appropriate - Contribute to reviews and service planning - Registered Nurse (NMC) active registration with no restrictions - Strong experience in complex care (ABI, spinal injury, neurological conditions) - Confident delivering handson clinical care in the community - Experience supervising or mentoring care staff - Excellent communication and documentation skills - Full UK driving licence Desirable Experience - Community or homebased complex care - Supporting multiple clients/packages - Experience with both adult and paediatric complex care - Knowledge of MDT working / case management - Meaningful Work: Make a real difference for individuals with complex needs. - Strong Clinical Support: Work closely with our Clinical Lead and wider MDT. - Career Development: Ongoing training, CPD and opportunities to progress. - Supportive Culture: Be part of a collaborative, valuesled team.
