Advanced Nurse Practitioner (ANP) Primary Care As an Advanced Nurse Practitioner, you will manage patients presenting with same-day and urgent clinical needs. You will work autonomously within a clearly defined scope of practice using your advanced clinical reasoning, diagnostic, and prescribing skills to deliver efficient, safe, and high-quality care under the professional supervision of our Chief Medical Officer (GP), with access to peer ANP and ACP networks for case discussion, reflective practice, and supervision. Youll assess, diagnose, and manage undifferentiated presentations (largely acute minor illnesses), including identifying red flags and initiating appropriate treatment or referrals. Main duties of the job Clinical Practice - Provide rapid access same-day and acute illness consultations (15-minute appointments). - Assess, triage, and diagnose undifferentiated and complex presentations. - Undertake advanced clinical assessment, including history-taking, examination, and diagnostic interpretation. - Identify red flags and escalates or refer appropriately. - Initiate and manage treatment, including prescribing and non-pharmacological interventions. - Deliver evidence-based care aligned with NICE and local pathways. - Provide safe follow-up, safety-netting, and patient education. - Support and guide clinical colleagues, including First Contact Practitioners and nursing staff. Evidence, Research, and Development - Contribute to service development and quality improvement projects. - Act as a clinical role model within the MDT, supporting professional development and reflective practice. - Record activity and outcomes in line with reporting requirements. - Support education and CPD supervision for multidisciplinary colleagues. - Maintain and evidence an ongoing portfolio aligned with the HEE Four Pillars of Advanced Clinical Practice. - Engage in continuous reflection, peer supervision, and CPD to sustain capability at an advanced level. Were looking for a highly skilled Advanced Nurse Practitioner to join our innovative, multidisciplinary team. Working under the professional supervision of our Chief Medical Officer (GP), youll be the first point of contact for adult patients presenting with acute illness or undifferentiated symptoms - helping people access timely, effective, and person-centred care within a 15-minute appointment model. Youll work alongside GPs, Physiotherapists, Clinical Pharmacists, Occupational Therapists, Podiatrists, Social Prescribers, and other allied professionals to offer evidence based joinedup care that puts patients first. Experience, knowledge and qualifications - Youll be a registered Nurse with the NMC. - Youll hold a Masters degree (MSc) in Advanced Clinical Practice or equivalent that meets the 4pillar framework (clinical practice, leadership, education, research), demonstrating capability through 1 of these routes: - Centre for Advancing Practice accredited MSc or ePortfolio (Supported Route) with Advanced Digital Badge; or - Currently enrolled on an accredited MSc Advanced Practice programme, with guaranteed progression; or - Enrolled on and progressed beyond the Learning Needs Analysis (LNA) stage of the ePortfolio route with supported progression to ANP role upon completion. - Youll have an Independent Prescribing (V300) qualification - Youll be able to demonstrate proven experience of applying advanced clinical reasoning and diagnostic skills across a broad range of presentations in practice. - Youll have excellent communication, triage and consultation skills. - Youll be able to work independently while contributing to a wider MDT. - Youll have an uptodate CPD portfolio demonstrating that you have knowledge of the Primary Care and General Practice Nursing Career and Core Capabilities Framework and able to demonstrate your ability to operate at advanced level practice. - You have experience working within or alongside a GP practice (desirable). This role profile is not exhaustive and you may be directed to complete other tasks as required. These duties will always be reasonable and deemed within the expectations of your position.
