Primary Care Clinical Pharmacist Care Bridge Clinic Cowichan PCN Island Health (DJS) - Duncan, BC V9L 1W7 CA (Primary) Job Description In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority, patient safety is a priority and a responsibility shared by everyone at VIHA; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position. Situated within identified primary care network locales, this position provides clinical pharmacy services to patients of the primary care network. As a member of an interprofessional team, this position works directly with patients to identify and resolve actual and potential drug therapy problems through comprehensive medication management. Travel may be required to multiple sites. Transportation arrangements must meet the operational requirements of the Vancouver Island Health Authority and may require the use of a personal vehicle. Shift: 8am to 4pm (operational needs may vary). Weekly hours: 75 biweekly. Start date: March302026. Salary: $51.37$64.19 per hour (fulltime). Fulltime (75hours biweekly). CRC required for professional registration. Qualifications - Licensed by the College of Pharmacists of British Columbia - Two (2) years recent related patientcare experience in a teambased primary care setting - Valid Class5 Driver's License An equivalent combination of training and experience may be considered.
