What We're Looking For? We are seeking enthusiastic Consultant Clinical Pharmacist (Long-Term Care and Assisted Living) who possess the following skills: - Strong knowledge of geriatric pharmacotherapy, federal and provincial pharmacy regulations, provincial long term care regulations, and electronic health records (EHR). - Excellent interpersonal and communication skills for working with a diverse healthcare team. - High attention to detail regarding medication orders and documentation. - Ability to work independently and manage multiple, complex patient cases. Key Responsibilities: - Perform comprehensive medication regimen reviews for residents, identifying, preventing, and resolving drug-related problems. - Collaborate with residents, family members and healthcare team to optimize patient outcomes. - Monitor for effectiveness of therapy and adverse drug reactions and recommend dosage adjustments as necessary based on clinical parameters. - Ensure compliance with provincial and federal regulations by auditing medication storage, documentation, and destruction in accordance with facility and corporate policies. - Participate in professional advisory committee or quality assurance committees to improve medication use process. - Act as a clinical resource for safe medication administration, potential side effects, and safe storage. - Educate patients and families on medication regimes. - Other duties as assigned. - Competitive wages - Comprehensive health and dental benefits - License and malpractice insurance reimbursement - Opportunity for career growth and professional development support Qualifications: - Doctor of Pharmacy (PharmD) or BSc Pharmacy degree from an accredited institution. - Previous experience in long-term care or geriatric pharmacy is preferred.
