Position The Behaviour Support Transition Lead (BSTL) and Clinical Coordinator is an integral member of the Virtual Behavioural Medicine (VBM) team at the Toronto Rehabilitation Institute. The role supports behavioural assessment, care planning, communication between VBM and Specialized Dementia Unit, liaison with Baycrest, program policy implementation, workflow improvement, and quality monitoring. The position is temporary fulltime (12 months, ending approximately May 2027) and reports to the Manager, Program Services. Responsibilities Provide behavioural assessment and develop patient care plans during admissions and discharges from the Specialized Dementia Unit. Coordinate patient care and communicate with patients, families, and interprofessional team members. Support the development and implementation of program policies and procedures. Improve clinic workflows and implement organizational initiatives regarding quality improvements and program development. Monitor program quality and performance; plan, organize, direct, and coordinate clinical activities related to patient care. Act as liaison between Baycrest and the Specialized Dementia Unit. Qualifications Minimum Bachelors Degree in a regulated health profession or recognized equivalent. Minimum 2 years clinical experience in geriatric rehabilitation, longterm care, dementia care or seniors mental health. Current Certificate of Registration in good standing from a college recognized under the Regulated Health Professions Act (e.g., College of Occupational Therapists of Ontario). Basic Cardiac Life Support (B Level or C Level) or Heart Saver Plus Canada. Experience working with medically complex frail elderly with cognitive impairment. Experience as member of an interprofessional team. Excellent interpersonal and communication skills (verbal and written). Excellent problemsolving skills. Compliance with the legislation and Code of Ethics and Rules of Conduct of the Canadian Occupational Therapy Association. Demonstrated competence with departmental and hospital safety, emergency and infection control procedures. Ability to exercise sound clinical judgment and function independently.
