Position Summary Dietitian, Diabetes Education Centre (DEC) The Registered Dietitian is a key member of the North York General interprofessional health care team, providing safe, effective, and ethical nutritional care for patients and families. You deliver care in accordance with professional and hospital standards, working both independently and collaboratively. You bring strong leadership, communication, critical thinking, and organizational skills while maintaining accurate clinical documentation. This position also supports the hospitals academic mandate and helps ensure a healthy and safe environment for patients, staff, and families. The Diabetes Education Centre (DEC) provides interprofessional care for pediatric and adult patients with type1, type2, and gestational diabetes, and you will apply indepth diabetes expertise to support patients, families, and fellow health professionals. Working collaboratively within an interdisciplinary team, you will deliver culturally sensitive, patient and familycentered care focused on assessment, education, prevention, and health maintenance. You will build strong relationships across the organization, demonstrating excellent communication and the ability to manage diverse patient needs. This position supports the hospitals academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation. On a practical level Provide comprehensive, patient and familycentered assessments and care, addressing emotional, physical, cognitive, and social needs for individuals living with diabetes and their families/caregivers. Deliver diabetes selfmanagement education and nutrition counseling to individuals, groups, families, and caregivers across all ages. Provide diabetes education to other health care professionals, consult with other health care teams and implement evidencebased practices. Develop and implement individualized nutritional care plans and therapeutic diet prescriptions in collaboration with interprofessional teams, following best practice guidelines. Advocate for the patient/family and community to ensure the patients rights are recognized and appropriate services are created/provided in collaboration with the interprofessional team. Coordinate and facilitate discharge planning with patients, families, and the health care team, ensuring effective use of relevant community resources. Lead and participate in centrespecific and corporate diabetes care initiatives, promoting evidencebased practices aligned with organizational strategic priorities. Promote best practice and evidencebased care through the implementation of initiatives aligned with the organizations strategic direction. Support in the development of program/unit goals, objectives, policies, procedures and quality assurance programmes. Provide dietitian coverage for the Centre of Complex Diabetes as required. Qualifications Successful completion of a degree in human nutrition and dietetics from a university program accredited by the Partnership for Dietetic Education and Practice (PDEP) or recognized equivalent. Successful completion of an accredited dietetic internship or equivalent supervised practical training. Member in good standing with the College of Dietitians of Ontario. Certified Diabetes Educator preferred. At least 1 year of recent clinical experience and expertise in diabetes management and education with a focus in Type1, Type2, and Gestational diabetes preferred (e.g., Type1 Diabetes, Insulin Pump Management). Certified Insulin Pump Trainer preferred. Membership with Dietitians of Canada preferred. Current teaching experience and counseling background. Current experience with electronic documentation and computer proficiency. Bilingualism in French/English an asset.
