Learn more at shn.ca Job Description Reporting to the Manager of Home Dialysis, the Home Dialysis Registered Practical Nurse (RPN) is responsible for providing support to the interprofessional Chronic Kidney Disease (CKD) team in readying patients and families to do hemodialysis treatments independently at home. Primary focus will be on providing education, encouragement, and training to patients and families as they embark on doing their own dialysis treatments at home. Clinical responsibilities of this position include but are not limited to daily care delivery, training, assessment, and support of home hemodialysis patients and families; reporting and collaborative care planning with primary nurses and the interprofessional CKD team, and dialysis machine management related to dialysis treatment delivery. Applicants must have experience in Hemodialysis. Requirements Current registration with the College of Nurses of Ontario Current BCLS Demonstrated clinical expertise in Hemodialysis and/or Peritoneal dialysis CNA Certification in Nephrology required or completion of Nephrology Nursing Certificates Understanding of the principles of adult education and demonstrated ability to apply those principles Minimum of 5 years of Nephrology nursing experience in working in Home Dialysis providing Peritoneal Dialysis and Hemodialysis care Previous experience in the area of patient education an asset Clinical knowledge and understanding of the CSN,KDOQI, KDIGO and ISPD Clinical Practice Guidelines Proven excellent interpersonal and communication skills both written and verbal Demonstrated excellent problem-solving, decision making and critical thinking skills Demonstrated excellent organizational, time-management and team building skills Driving License Demonstrates values consistent with SHNs Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation.
