Empower patients as a Registered Practical Nurse specializing in Home Peritoneal Dialysis. Provide essential training and education to help patients achieve independence in their treatments at home. In this pivotal role, you will collaborate with the interprofessional Chronic Kidney Disease team. Your clinical expertise in Hemodialysis will ensure patients and families receive top-notch support as they transition to home care. This position requires ongoing assessments and collaborative planning to enhance patient experiences and outcomes. Key Responsibilities: Educate patients and families on home dialysis Assess and manage daily home dialysis care Collaborate with CKD team for patient planning Ensure effective training on dialysis equipment Report patient progress and challenges diligently Requirements: Current registration with Ontarios Nursing College 5+ years in Nephrology nursing experience Expertise in Hemodialysis and Peritoneal dialysis CNA Certification in Nephrology required Excellent communication and problem-solving abilities Become a vital part of patient journeys, guiding families in achieving confident, independent home dialysis management. #J-18808-Ljbffr
