Actively participates as part of the program and professional leadership team in promoting quality care by implementing evidence based practices, evaluating patient outcomes, development and evaluation of policies, clinical programs and services. Models both the professional and organization core competencies and provide expert clinical nursing leadership.**What youll be doing:**Advanced Clinical Practice (75%): Within a model of collaborative practice, the APN/ACNP uses advanced theoretical knowledge base and clinical competence and skill to assess, plan, provide, and facilitate care for a specific population of patients, families, or groups across a continuum of care.* Assesses patients/families through multiple strategies, including comprehensive admission history and interview, advanced physical examination, interval assessments and the interpretation of diagnostic testing to determine need for medical, nursing, or other care.* Utilizes the nursing process to determine patient response to their health state.* Utilizes diagnostic reasoning and decision making process to formulate a list of hypotheses, determine the need for, order and interpret diagnostic testing, and formulation of a medical diagnosis within the specialty scope of practice and consistent with practice guidelines.* Communicates significant clinical findings to the attending physician, and facilitates ongoing communication of patient progress and plan of care to the health care team.* Initiates/orders, tailors and evaluates/discontinues selected medical and nursing therapy (including pharmacologic therapy) within the specialty scope of practice and consistent with practice guidelines and established medical directives for APN/ACNP to optimize the plan of care.* Performs specialty-specific diagnostic and therapeutic interventions in accordance with scope of practice, approved medical directives for controlled acts and established practice guidelines.* Facilitates patient care by consulting with the patient/family and multidisciplinary patient care team to formulate, initiate, co-ordinate and evaluate a comprehensive and collaborative patient centered plan of care, ensuring effective and efficient use of resources.* Promotes continuity of care within the hospital and between the hospital and community by initiating and co-ordinating referrals for consultation for hospital and community personnel and agencies, and formulating, implementing and evaluating patient discharge plans.* Writes/dictates admissions, consultations, progress notes, and discharge summary reports.* Evaluates patient care outcomes and nursing practice issues to ensure the highest possible quality of care.Education (10%): Provides leadership in professional and patient population learning and education by:* Developing teaching programs and conducting educational teaching sessions with patients/groups and or families, in hospital or in community/public forums with focus on health promotion, illness prevention, and the impact of the illness experience on patient and family.* Providing leadership in the assessment of learning needs, development and evaluation of patient education resources and programs for specific patient populations.* Facilitating and providing educational opportunities and programs, in collaboration with the Nurse Educator, for nursing, medical and allied health students and staff for the enhancement of specialized knowledge and skills related to the specialty area.* Supervising/precepting nursing students (graduate and undergraduate) and other interdisciplinary students as required in their clinical experience and evaluates their performance.* Providing clinical nursing leadership to promote professional development, to assist nursing staff in the application of knowledge/theory to practice through role-modeling and coaching, an by participating in formal staff education programs in collaboration with the program leadership team.* Engaging in joint activities with Faculties of Nursing, and other academic faculties and organizations through collaborative projects, research and cross appointments.Research (5%): Contributes to the body of nursing/specialty knowledge by:* Providing leadership in the critical analysis and review of literature and the application of research findings to practice through the development and implementation of clinical practice guidelines and standards of care.* Identifying and implementing strategies to translate and interpret research findings and theoretical frameworks into practice.* Facilitating the utilization of research by nursing and multidisciplinary staff and contributing to knowledge base through presentation or publication.* Facilitating or conducting clinical research independently or collaboratively to monitor and improve patient care practice.* Evaluating the effectiveness of nursing components of clinical programs including the nurse practitioner roleOrganizational and Professional Leadership (10%): Utilizes advanced preparation and knowledge of patient population needs, processes of patient care, care systems, the research process and professional practice to provide and support leadership in the following areas:* The development, implementation, and evaluation of quality management programs/initiatives, care delivery models, practice standard/guidelines/policies and procedures and associated health outcomes and resource utilization, in collaboration with the professional practice and nursing leadership teams.* Serving as a resource or consultant to administration in decision-making, strategic planning and problem solving processes, and ensuring the integration of advances nursing practice in program planning, policy making and problem solving.* Identify patterns and trends in health care and patient care outcomes and develop alternate approaches in relation to patient care situations.* Participation in hiring, supervising and performance appraisal of nursing, administrative and other multi-disciplinary staff.* Participation and representation of advanced nursing practice on committees relevant to patient care issues at all levels of the organization.* Active participation on internal and external professional organizations and committees.* Dissemination of nursing knowledge through presentations
