Detailed Overview In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, and working collaboratively with the interprofessional care team in the provision of personcentered care, we provide and coordinate pre and postoperative nursing care including assisting with surgical and interventional procedures and discharge teaching for adults and children in an inpatient unit and/or outpatient clinic or setting. Responsibilities Assesses patients and collects information from a variety of sources using observation, communication, data review and physical assessment; makes a nursing diagnosis; determines whether an appropriate intervention can improve the condition; plans, performs and evaluates nursing interventions. Observes patient acuity for significant changes; intervenes to provide and manage care; resolves problems or emergency situations by planning and performing interventions and evaluating outcomes. Assists physicians with therapeutic interventions by positioning patients, using procedurerelated equipment, and maintaining all procedural equipment. Collaborates with patients and families in developing the care plan and encourages family participation; establishes a therapeutic relationship by demonstrating empathy, trust, respect, and advocacy for patient rights and autonomy. Participates in the care and discharge planning process for admitted patients, ensuring effective implementation, updating the plan, and collaborating with the interprofessional team and Patient Care Coordinator. Coordinates nursing care; sets priorities, delegates tasks to support staff, and makes referrals to the interprofessional team per protocols and practice standards. Collaborates with the interprofessional teamincluding physicians, LPNs, Patient Care Coordinators, Clinical Nurse Educators, and other healthcare professionalsto identify and resolve issues, adjust care plans, and address changes in patient condition. Teaches patients and families about illness prevention, health restoration, and health maintenance; assesses learning needs and demonstrates care activities to support selfcare and discharge planning. Plans and discusses referral information with patients and families, and, together with the interprofessional team, initiates referrals to appropriate health care or community agencies. Documents assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes, and referrals according to practice standards and Fraser Health procedures. Participates in nursing committees, care conferences, and councils as assigned or agreed with the Manager. Participates in research and qualityimprovement activities by identifying patientcare issues, collecting data, and recommending corrective actions or changes to practices, procedures, or protocols. Participates in and attends inservices and other educational programs, identifies learning goals, and maintains current clinical competence within the area of practice. Acts as a preceptor or mentor to assigned new nursing staff and students; shares information, demonstrates care activities, and participates in orientation. Provides feedback to the Manager or delegate regarding the effectiveness of patientcare programs, policy or clinical decisionsupport tools, and equipment. Performs other related duties as required. Qualifications Education and Experience Graduation from an approved School of Nursing and current registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Two (2) years of recent relevant clinical experience in the designated clinical area and completion of postgraduate course work and/or certification applicable to the designated clinical area, or an equivalent combination of education, training and experience. Skills and Abilities Ability to initiate and maintain intravenous therapy. Ability to complete initial and ongoing patient assessment and provide nursing care through appropriate technical and therapeutic interventions. Ability to communicate effectively with patients, families, the public, medical staff, and interprofessional team members using verbal, written, and electronic means. Ability to promote personcentered care that demonstrates empathy and sensitivity to diverse cultures and patient preferences, and to advocate for patients. Ability to integrate and evaluate data from multiple sources to problemsolve effectively. Ability to teach and instruct patients and families on essential healthcare topics. Ability to organize and prioritize work. Ability to collaborate and promote cooperation among interprofessional team members. Ability to use evidencebased practice to support nursing decisions and actions. Ability to operate relevant equipment and computer applications.
