In this role, you aren't just a clinician-you are a trusted partner to families and a key collaborator in the healthcare journey to ensure safe, effective and meaningful care.This role is ideal for a compassionate, innovative, and collaborative professional who thrives in a dynamic home and community care setting.What Youll DoClinical Responsibilities:Provide quality nursing care, within the scope of practice and in accordance with the CNOMaintain knowledge and compliance to the Professional StandardsFollow nursing processes and practices in an effective and efficient mannerEnsure the maintenance of high-quality nursing skills, applying current knowledge to nursing careComply with all SJHC nursing practices, processes and work instructionsReview and followthe plan of nursing care, including discharge plansEngage in consultation / liaison with clients, their families/caregivers, physical, multi-discipline team member, and other external stakeholders as required (eg. HCCSS, ICC Care Coordinators)Evaluate and monitorcare plans on an on-going basisAssist other clinical programs if requested or requiredParticipate in regular and on-going professional development / learning opportunities, with the goal of obtaining more advanced or specialized nursing skills, as may be necessary for organizational advancementComplete self-appraisals in keeping with the College of Nurses of Ontario Standards of Practice and SJHC internal practicesAct as an advocate for patients, families and their caregivers through reviewing and interpreting information, supporting decision making and facilitating communicationsComplete comprehensive nursing assessmentsConduct virtual care visits as necessary to program and/or scope of practiceOperational:Complete program, clinical or organizational documentation in a timely and accurate mannerParticipate in organizational learning opportunities as appropriate (e.g, Surge Learning)Engage in organizational working groups as appropriate (e.g, JH&SC, Quality Committee)Effectively utilizeorganizational tools, software, devices in the application of daily work, willingness and ability to learn new technical skills as operational or patient needs changeWillingness and ability to provide care in a variety of geographical locations (eg. Central Hamilton), patient care environments (eg. apartment building), or institutional care settings (eg.
