Job Title

Summary As a Home Health Scheduler you will coordinate/assign visiting disciplines to patients, including but not limited to nursing, physical therapy, occupational therapy, speech language pathology, medical social services, dietician, and home health aides. You will coordinate schedules between multiple providers and disciplines to assure optimal patient care, efficient utilization of services, and compliance with physician orders.
