The Clinical Consultant Pharmacist is responsible for maintaining continuity of care for clientele through collaboration between onsite physicians, nursing staff, and other health providers at long term care, supportive living, and other congregate living communities. The Clinical Consultant Pharmacist is responsible for completing clinical pharmacy activities in alignment with the Continuing Care Health Service Standards, site policies, internal CareRx policies, and the Ministerial Order for Clinical Pharmacy Services. Role Accountabilities - Act as aliaisonbetweenlong term care homesand retirement communitiesand CareRx Pharmacyto ensure optimal Resident care, Pharmacy Services, and Clinical support - Travel to LTC homes and retirement communities - Build and maintain relationships within the community to ensure ongoing collaboration and communication between home/communityleadership teams and pharmacy teams to develop/maintain strong partnerships - Complete medication reviews in currently serviced retirementcommunities and long-term care homesserviced by CareRx Pharmacy - Complete all required documentation and billing processes as required by MOHLTC, College of Pharmacists of British Columbia and other federal and provincial standards and regulations - Provide education to Residents and staff on various health-related topics including theMeds Checkprogram and attend community-based wellness fairsand other eventsas required - Conducteducation sessions withinLong Term Care Homes and Retirement Communities withinscope of practice, as applicable (e.g. policy and procedure, web portal,eMAR, influenza protocol, etc.) - Support CareRx Pharmacy policies and procedures within retirementcommunities and long-term care homes - Completemedication audits on-site in LTC Homes and Retirement Communitiesas required - Ensure regular on-site visits are made to all assigned clients - Attend/participate in annual professional conferences and trade shows, when applicable - Participate in the development and implementation of industry-leading clinical pharmacy service, when applicable - Be an education, policy and pharmacy initiative resource - Be available for special clinical team projects as required - Complete/collaborate in the develop of medication audit follow-up improvement plans when applicable - Works collaboratively with other team members to support applicable homes and communities - Participate in pharmacy-related transition processes (e.g., new client transitions, eMAR transitions, etc.) - Help with pharmacy operations/drug distribution when required - Attend and actively participate in clinical team meetings What You Will Bring to The Team - Insured and currently licensed in good standing with the College of Pharmacists of British Columbia. - Proven ability to foster strong partnerships with other members of the healthcare team - Effective organization, planning, and time management skills - Knowledge of Kroll Windows applicationand Microsoft Office programs - Completeunderstandingofprovincial regulations governing LTC homes and Retirement Communities - Must be familiar with the provincial standards of practice established for LTC pharmacies - Excellent customer service and interpersonal skills, oral and written communication skillsand be able to interact with persons of varying levels of professional expertise - Skilled in critical thought processes and be able to prioritize appropriately - Must demonstrate initiative and creativity - Self-directed requiring minimal supervision and able to work independently - Motivated to achieve results that reflect a high standard of quality - Good listening skills and be adaptable to changes in job environment - Dependable and responsible in the execution of job duties - Adaptable to changes within internal work structures - Strong presentation and teaching skills - Must possess a valid drivers license Employee Testimonial 'One of the things I love about the Clinical Consultant Pharmacist role is the focused time I get to spend with the residents, their families, and the health care team, which helps me better manage their medication therapy, resulting in improved quality of life and health outcomes.' Compensation Range $110,000-$120,000 annual salary pro-rated over a 3-day work week. (Actual pay rate will be based on the individual candidates credentials and experience.) Location Location will primarily be LTC homes and retirement communities in the Kelowna and Kootenays surrounding area such as Cranbrook, Kimberley, Invermere, Golden, Creston. Work Shifts This is a part-time position working 3 days a week, with flexibility and opportunity to work-from-home and on-site from the care homes. Opportunity This is a current existing position.
