The Senior Manager, Integrated Care Programs, is accountable for supporting and evolving Home Health Integrated Care Programs through developing and fostering strong internal and external relationships. Acting as a liaison between external partners and internal departments, the Senior Manager of Integrated Care Programs is vital to the sustainability and growth of existing programs as well as to the implementation and oversight of new programs. This role is part of the integrated care design team, helping to build innovative approaches while supporting a culture of excellence between CBI Home Health and our valued partners. Our Total Rewards offerings include: Competitive salary: $95,000 to $105,000 per year (The salary offered will depend on several factors such as relevant skills, qualifications, and experience) Hybrid working environment, which allow worklife balance and focus on quality of life Comprehensive benefits and rewards package, including group health, dental and vision benefits, 100% employer paid Employee Assistance Program Career growth and leadership development opportunities What you will do: Act as the primary point of contact for community partners (funders, vendors and subcontractors) for integrated care programs Participate in program operational codesign working groups with internal and external stakeholders Maintain a solid understanding of stakeholder needs and gaps (potential opportunities) with integrated care programs to ensure CBI Home Health is well positioned to respond with innovative solutions Collaborate with partners to identify solutions for identified gaps and issues Ensure accurate data collection and reporting of key performance metrics for each integrated care program Identify opportunities for program improvement and successes based on data and trends Collaborate with CBI Home Health local operations and corporate supports to ensure a high performing integrated care program Identify opportunities for program improvement and engage stakeholders to implement changes. Identify partnerships and community resources to enhance existing programs to support effective and sustainable transitions Support the confirmation and mobilization of resources to effectively support patient transitions to the community Collaborate with Operations to ensure integrated care roles are filled with attention to role fit based on Key Accountabilities What you will need to be successful: Regulated Health Professional registered and in good standing with the respective College Postgraduate education such as an MBA or advanced university/executive training considered an asset 3 years experience in a general management role ideally in a healthcare sector, home health care preferred Demonstrate the ability to lead teams and individuals to achieve results Demonstrated success with program evaluation and improvement Experience with clinical program monitoring, and financial management Possess excellent communication and organizational skills, with the ability to work as part of a team in a fastpaced environment Have strong knowledge of quality improvement strategies, health care system, and experience in home and community care Be comfortable interacting with senior level stakeholders within partner organizations Combine a demonstrated patientcentered focus with a commitment to providing quality care Recruitment Technology Disclosure: We use technology and automation to support administrative aspects of our recruitment process.
