Nelson - Regular Community Disability Practitioner Job Description Our group home in Northwest Calgary seeks a Community Support Worker professional to support an array of individuals with complex behaviors and disabilities. Your role will involve motivating individuals toward independence and meaningful community participation. Individuals you support may have diagnoses such as FASD, Borderline Personality, ADHD, Addiction, Complex Trauma, Schizophrenia, mood disorders, and other developmental disabilities. You will follow individualized support plans to help them achieve their goals. Tasks may include personal care, gtube feedings, driving to medical appointments, and daily activity assistance. Key responsibilities include: Providing medical, technological, environmental, emotional, physical, and behavioural support Attending medical/legal appointments with the individuals Administering medication and completing daily documentation and key worker duties Assisting with daily chores and activities in the community or group home Maintaining safety, cleanliness, and hazard assessments of individuals living spaces Communicating with OPG/Guardians/Family Members Advocating and researching support options (e.g., hospitals) when necessary Providing specific support for individuals on ODP or O.A.T. programs Driving to meet individuals and transporting them to activities or appointments Learning behavioural support plans to better understand individual needs Completing incident reports as required About you: You are dedicated to learning and researching ongoing behaviors and concerns of the individuals. You have experience or knowledge in Harm Reduction Philosophy & Trauma-Informed Care Practices, current mental health and addiction concerns, and biohazard or illicit substance use. You are willing to receive training in Universal Precautions or Infection Prevention & Control, support safe disposal of needles or paraphernalia, and collaborate with the team on trauma histories and strategies. You exhibit leadership, adaptability, and resilience in crisis situations. Position: Permanent Schedule: Sat, Sun 3:00 PM to 11:00 PM Wednesday 7:00 AM to 3:00 PM Rate of Pay: $21$23 per hour Additional compensation: mileage reimbursement $0.50/km; 4% vacation pay; 6 flex days per year after 3 months of employment; eligibility for casual hours at other locations.
