Location: 2300 Eglinton Avenue West and 2000 Credit Valley Road, Mississauga Reports To: Operations Manager Organization Overview Credit Valley Family Health Team (CVFHT) is an academic Family Health Team affiliated with Trillium Health Partners and the University of Toronto. We are a collaborative, interprofessional primary care organization committed to continuous learning, quality improvement, and excellence in patient care. As a notforprofit organization funded by the Ministry of Health, CVFHT provides comprehensive primary care services to over 15,000 patients in Mississauga, including a dedicated mandate to serve the Francophone community. Role Summary The Registered Nurse (RN) provides comprehensive, patientcentered nursing care in accordance with the standards of the College of Nurses of Ontario. Working within an interprofessional team, the RN is responsible for assessment, care planning, implementation, and evaluation of patient care, while actively contributing to quality improvement, health promotion, and program development initiatives. Key Responsibilities Clinical Care and Patient Support Conduct comprehensive and ongoing patient assessments to identify health needs, risks, and priorities Develop, implement, and evaluate individualized care plans in collaboration with patients and the care team Provide health education, counselling, disease prevention, and health promotion across the lifespan Deliver clinical interventions including (but not limited to): immunizations, injections, wound care, ear syringing, ECGs, spirometry, wart treatment, and ambulatory blood pressure monitoring Perform telephone triage and followup within scope of practice Support chronic disease management, wellbaby care, and family health Maintain accurate, timely, and complete clinical documentation Collaborate with patients, families, physicians, and interprofessional team members in care planning and delivery Coordinate care and facilitate referrals to internal and external providers as appropriate Liaise with hospitals, community agencies, and other partners to support continuity of care Ensure smooth transitions across care settings Education, Research & Quality Improvement Provide teaching and mentorship to Family Medicine residents, nursing students, and other learners Participate in research and evaluation activities aligned with FHT priorities Contribute to quality improvement initiatives and ongoing evaluation of clinical programs Identify opportunities to enhance patient experience, access, and outcomes Collaborate with leadership to design and implement programs aligned with population health needs and system priorities Support development and evaluation of services using patientcentered and codesign approaches Participate in program planning, implementation, and evaluation activities Contribute to partnerships with community and system stakeholders Support change management initiatives and continuous improvement efforts Participate in committees, working groups, and organizational initiatives as required Perform additional duties as assigned by leadership Qualifications Bachelor of Science in Nursing (BScN) Current, unrestricted registration with the College of Nurses of Ontario Minimum 3 years of clinical experience in primary care (Family Health Team experience preferred) Fluency in both English and French (written and verbal) is considered an asset Other assets: experience in chronic disease management, familiarity with electronic medical records (EMR) systems and experience in group facilitation and patient education Core Competencies Strong clinical assessment and critical thinking skills Excellent communication (oral and written) in both English and French Ability to work independently and collaboratively within an interprofessional team Strong organizational, time management, and prioritization skills Adaptability and ability to work in a fastpaced environment Proficiency in Microsoft Office applications Benefits CVFHT offers a competitive salary and comprehensive benefits package, including HOOPP pension, extended health and dental coverage, and professional development opportunities.
