Salary: CAD $41.42/Hr $55.91/Hr. Responsibilities Community Health Nurse works within an interdisciplinary teambased care model, collaborating with clients, families, and caregivers. The role involves assessing, planning, coordinating, organizing, implementing, evaluating, and transitioning nursing and other services for clients with acute, chronic, palliative, and rehabilitative needs. The nurse delivers direct care, manages care plans, facilitates client selfmanagement, and navigates community resources. Qualifications Registered Nurse, in good standing with the British Columbia College of Nurses and Midwives. One year of recent related clinical experience with complex clients, chronic health conditions, or functional mobility issues, including care and transition planning. Valid BC Drivers License. Knowledge, Skills & Abilities Community health nursing theory and practice in a client and familycentred model. Clientfocused, traumainformed, harmreduction, and culturally safe assessment and intervention. Critical thinking and risk assessment in home environments; integration of data from multiple sources. Conflict resolution, behavioural management, and endoflife care transition skills. Prioritisation, independence, creative planning, and implementation of policies. Resiliency and adaptability to diverse cultures and changing situations. Collaboration with interdisciplinary teams and family members. Accurate documentation in an electronic medical record and proficiency with related software. Confidentiality and privacy practices. Effective communication across verbal, written, and electronic media. Teaching, facilitation, and coaching of clients and families. Knowledge of provincial acts, regulations, and homecare policies. Understanding of acute, chronic, palliative, mentalhealth, and substanceuse care. Awareness of community resources and support services. Knowledge of research, quality improvement, evaluation methods. Planning, organising, scheduling, and prioritising work. CPR skills. Use of nursing equipment and techniques.
