Employment Status Temporary Full Time. Duration (if contract): October 30, 2026 Hours of Work Monday - Friday, 8:00am 4:00pm. Location Humber River Health, Finch Reactivation Care Centre Employee Group ONA Hourly Rate $45.49 $63.32 Reporting Relationship Patient Care Manager Responsibilities Clinical Practice Ensures accurate and complete communication regarding patientrelated issues from shift to shift, assists manager/educator in identifying any areas of clinical practice that need development, and ensures a safe environment for patients, visitors, and staff. Ensures appropriate followup of any issues arising from the previous shift (e.g., patient/family concerns, incidents, changes in patient acuity). Has general awareness of plans of care for all patients and, in consultation with staff, ensures patient assignments are based on patient needs, scope of practice, and skills of staff. Patient Care Manages the daily flow of patients through the Cancer Clinic. Adjusts staffing and assignments throughout the shift as required and anticipates adjustments for future shifts. Acts as a clinical resource to staff as needed. Triage Cancer Clinic referrals to oncologists according to the decisionmaking matrix approved by the oncology physician team. Facilitates effective communication among interdisciplinary team members, reports problems affecting patient care or the unit to the manager, and delivers direct patient care as required. Telephone triage of patient symptomrelated phone calls. Collaborates with Resource Persons on other units/areas regarding patient transfers and clinical issues. Liaises with other departments, the Home and Community Care Coordinator, and other community stakeholders (e.g., private oncologist offices, referral offices, external hospitals) regarding clinical issues. Educational Responsibilities Engages in continuous learning activities such as attending inservices, seminars and courses, and shares information obtained at educational sessions with colleagues. Assists the Manager/Clinical Practice Leader in identifying staff educational needs in conjunction with the Leader, coordinates staff attendance at inservices, and shares nursing knowledge and expertise with staff through incidental teaching. Collaborates with nursing staff and interdisciplinary team members to ensure patient teaching programs are implemented. Identifies the need for patient education resources/materials. Participates in the nursing education program by collaborating with the Manager/Clinical Practice Leader and instructors from nursing faculties to help identify preceptors and appropriate patient assignments for students/orientees. Facilitates orientation of orientees/students. Assigns staff to attend mandatory inservices. NonClinical Responsibilities Informs manager of any relevant issues related to patients, families, or staff. Leads daily Patient Quality & Safety huddles with the interdisciplinary team. Ensures staff provide factual, timely information/documentation to the Manager regarding patient/staff incidents or equipment malfunctions. Ensures equipment on the unit is maintained in good working order and participates in continuous quality improvement. Ensures safety issues are dealt with promptly by initiating maintenance requisitions. Assists the Manager with staffing (e.g., replacement for sick calls, schedule changes) and ensures an appropriate composition of staff to meet patient care needs in the upcoming weeks. Represents the unit on oncology committees (e.g., PFAC, Integrated Cancer Care). Other duties as assigned by the Manager. Requirements Baccalaureate degree in nursing preferred. Registered with the College of Nurses of Ontario in good standing. Maintains current speciality certifications for the related clinical area. BCLS certification or equivalent. Two years of recent/related experience in a clinical specialty, or equivalent education and experience within the last five years. Related CNA certification and specialty certification (if appropriate) from an accredited program preferred. Leadership experience.
