Position: GAIN Community Team Occupational Therapist Position Type: Full-time Permanent (On-site) Hours of work: 35 hours per week Compensation: $40 Hourly Date posted: May 20, 2025 Position Summary: The Occupational Therapist in the Geriatric Assessment Intervention Network (GAIN) Community Team is responsible for providing assessment utilizing various standardized tools with clinical judgment and developing interventions for older adults and seniors affected by emotional, psychological and physical impacts of aging. Responsibilities Participate as geriatric assessor in interprofessional Comprehensive Geriatric Assessment (CGA) Complete clinical triage for new referrals Data gathering and recording within all domains of Comprehensive Geriatric Assessment (CGA) Provide clinical care and consultation services to improve and/or maintain activities of daily living for frail older adults and seniors Analyze frail older adults and seniors capabilities and expectations related to life activities through observation, interviews and formal assessments Collaborate with the GAIN interprofessional team to establish care plans for patients Qualifications University degree in Occupational Therapy Registered and in good standing with the College of Occupational Therapists of Ontario Registration with CAOT (Canadian Association of Occupational Therapists) or OSOT (Ontario Society of Occupational Therapists) is an asset Current CPR and First Aid certification Two to three (23) years working experience in the field Demonstrated extensive knowledge and skill working with individuals with various types of dementia, delirium, mental health, addictions, responsive behaviours, other neurological conditions, and those who experience an acute behavioural change Experience and knowledge of working therapeutically with geriatric populations is an asset Demonstrated sound knowledge of cognitive and mobility assessment tools Certification in Gentle Persuasive Approaches, PIECES or Montessori preferred Skill at crisis intervention and assessment preferred Proficiency in other languages desired (Armenian, Greek, Mandarin, Tamil, Hindi, Urdu, etc.) Vulnerable sector check required A valid drivers license and a car required for home visits Benefits Competitive compensation and benefits Rewarding career that supports meaningful work in our communities Training and professional development opportunities Healthy and safe working environment Equal Employment Opportunity Senior Persons Living Connected is an inclusive and diverse work environment.
