Overview Reporting to the Director of Care, Long-term Care, the Associate Director of Care, Long-term Care (LTC), serves Kensington Health''s LTC home (i.e., Kensington Gardens) to support clinical operations and advance quality of care, safety, and the experience of the people we serve. The role is pivotal to ensuring operational excellence by stewarding human, financial, and material resources at Kensington Gardens. This role will be guided by the legislation and regulations governing LTC in Ontario, the organization''s policies and procedures, and best practices for nursing and personal care. As a key member of the operational leadership team, the Associate Director of Care, Long-term Care, will lead a team of registered nurses, registered practical nurses, personal care attendants to deliver daily care and services for the residents who live in their assigned resident home areas (RHAs) at Kensington Gardens. Availability Required: Days, Evenings, and Nights Shifts Compensation: $111,403.5 - $133,672.5 Responsibilities Human Resources Works with human resource team members to effectively recruit and retain registered nurses, registered practical nurses, and personal care attendants. Works in collaboration with the Health and Safety team to support team members who require modified duties, hours, and/or accommodations in the workplace. Monitors and manage team member performance in collaboration with the Director of Care, LTC, and Human Resources to resolve performance and/or attendance concerns. Completes regular performance evaluations for registered nurses, registered practical nurses, personal care attendants, and other assigned team members who work in the assigned RHAs. Collaborates with staffing coordinators to ensure all shifts are filled, addresses unplanned absences, and approves any changes to the staff schedule. Reviews and approves/declines time off requests. Supports continuous learning and professional development of team members through education, coaching, and mentorship. Fosters productive relationships with the union, including participation in labour-management meetings when indicated. With guidance from the Director of Care, Long-term Care, fosters a workplace free of violence and harassment and promotes workplace safety by taking action when concerns are brought forth or observed. Demonstrates a commitment to equity, diversity, and inclusion by championing initiatives that promote equity, diversity, and inclusion. Financial and Material Management Provides input via the Director of Care, Long-term Care, into annual operating and capital budgets for the LTC home. Makes financial decisions in alignment with their delegated signing authority with the goal of responsible financial stewardship. Ensures clinical equipment and supplies are well-maintained, inventoried, and there are sufficient supplies and equipment at the point of care for the assigned RHAs. Provides input into the minor and major capital equipment needs as requested. Ensures supplies and materials that are ordered and received are aligned to RHA needs and remain within budget. Clinical Operations Oversees clinical operations for assigned RHAs with a focus on the care and services provided by registered nurses, registered practical nurses, and personal care attendants. Ensures that the assigned RHAs are consistently staffed within the approved staffing model and budget. Liaises with colleagues on the admissions, transfers, and discharges of people living at Kensington Gardens as needed. Ensures the interprofessional care planning process is implemented for each resident living at Kensington Gardens, including coordination and execution of the initial and annual care conferences, and as needed, for each resident. Monitors the health status of residents in their assigned home areas and liaises with the interdisciplinary team members and resident/substitute decision maker as needed. Engages with attendant and consulting physicians and nurse practitioners to foster high-quality, timely care. Ensures that documentation in the health records meets professional and organizational standards for registered nurses, registered practical nurses, and personal care attendants. Ensures that team members are aware of and comply with organizational policies, practices as well as legislative and regulatory requirements. Regularly audits the health records of those living at Kensington Gardens with particular focus on residents with high-risk factors (e.g. falls, pressure injuries, responsive behaviours, etc.) and takes action to address any care or process gaps. Leads the implementation, monitoring, and annual evaluation of a clinical program that drives quality of care and improvement in outcomes as assigned (e.g., skin and wound, falls, continence care, pain management, medication safety, etc.). Quality, Safety, and Experience Fosters a service excellence culture through role modeling the values of the organization. Oversees and supports the resolution of inquiries from the resident who lives in the LTC home and their loved ones, including following the complaints escalation policy and fulfilling any notification and disclosure obligations as required. Supports the achievement of quality improvement goals and objectives (e.g., annual quality improvement plans) by leading and/or supporting the implementation of initiatives. Promotes a safety culture through timely reporting (internal and external), completion of safety incident reviews, and sharing of system and process learnings with their team. Promotes person-centred approaches to care and service through role modeling and fostering relationships with the Residents'' and Family Councils. Other Participates in committees and engagement forums as required by the organization. Required Skills and Abilities Patience, flexibility, and exceptional interpersonal skills. Proven conflict resolution and communication skills. Demonstrates strong critical thinking ability to make decisions and can articulate rationale for decisions. Ability to work effectively and demonstrate composure under pressure while meeting multiple and competing deadlines. Ability to discern relevant information, apply critical thinking, and facilitate the resolution of challenging situations using diplomacy, tact, and persuasion. Ability to establish effective working relationships. Demonstrates high level of initiative, self-direction, flexibility, and ability to adapt to changing situations. Required Knowledge and Experience Minimum 5 years of formal management roles in healthcare operations; Long-term Care sector preferred. Has critical assessment skills that aid in constructive problem solving, influencing, and resolving conflict. Demonstrated leadership in implementing initiatives related to safety and quality improvement. Proven track record of successful change management initiatives. In-depth experience related to clinical assessment, planning, and interventions related to the care of people with complex needs, and particularly those living in a long-term care setting. Knowledge of infection prevention and control principles and practices. Knowledge of applicable legislation and regulations (e.g., Occupational Health and Safety Act, Employment Standards Act, Fixing Long-term Care Act, Health Care Consent Act, Personal Health Information Protection Act (PHIPA) etc). Upholds strict compliance to reviewing and releasing confidential documents, especially those that include personal health information. Follows release of information processes that align with corporate policy and PHIPA legislation. Required Professional Designation/Certificate Bachelor''s degree in a Regulated Health Professional Field is preferred or equivalent experience. Master''s degree preferred (e.g., nursing, health administration, leadership). Active member in good standing with the Regulated College of Ontario.
