McMan is recruiting for a Case Manager to join our team providing a housing first and harm reduction approach; the Case Manager will be responsible for assisting residents in maintaining their housing, assisting accessing financial supports, supporting their education/employment goals and any other areas of support that is required to help them successfully transition into independence. The Case Manager will work alongside our Transition Specialists, by being a member of the residents team that is planning around the needs for their successful transition into the community. Key aspects of this role include: Educate and support individuals to increase skills that promote greater selfefficacy. Provide individual instruction, modelling, and support of basic life skills (e.g. health, nutrition, home cleanliness, safety). Monitor and support individuals through a harm reduction approach to ensure living conditions are safe and sanitary. Implement integrated service delivery from the point of referral to program completion and transition for individuals accessing the program. Complete case management responsibilities that include assessments, goal planning, case consultation, monitoring and evaluation of the goals to meet the clients needs. Required qualifications: A degree or diploma in Human Services. Three (3) years experience working with vulnerable populations. Strong writing, organisational, programme planning and interpersonal skills. Understanding of Rapid Rehousing. Demonstrated interest and knowledge of the Homeless Serving System of Care. Experience with HMIS data preferred. Current standard First Aid Certification and Suicide Intervention Training. Valid Drivers licence and adequate insurance coverage. The successful candidate must also have a Child Welfare Intervention Check and Police Check (current within 6 months). *This role will require oncall support on a rotating basis and some evening and weekend work hours.
