All Staff are Eligible for: Healthcare of Ontario Pension Plan (HOOPP) Employee & Family Assistance Program Paid Holiday Float Day Professional Development Opportunities CMHA-S/M Internal Job Posting 26-04 Housing Case Manager Department: Community Development & Housing Employment Status: Full-Time Contract, 12 months with possibility of extension Hours per Week: 70 hours biweekly Responsibilities Reporting to the Coordinator, Housing Case Management, the Housing Case Manager: Provides housing supports to individuals living with mental health and/or substance use concerns to reduce the risk of homelessness, crisis or hospitalization and improve well-being. Working within the framework of a client directed rehabilitation model, staff provide assistance to individuals in obtaining housing, enhancing instrumental activities of daily living skills and developing wellness strategies and maintaining their living environment. Housing Case Managers can provide support in areaspecific streams such as the justice system, homelessness sector and/or transitions from hospital. Qualifications Completion of a 2 year diploma or degree in a health or social services discipline. Combination of postsecondary education and 5+ years of experience that reflects the job description and requirements of the current position will also be considered. Minimum one year experience in related position. Fluency in English is essential. Advanced conversational French is essential for 40% of these positions. Priority will be given to candidates who are bilingual in French and English as per our French Language Service Plan. If no bilingual candidates (both internal and external) meet the qualifications, unilingual Englishspeaking candidates may be considered. Access to a reliable vehicle required to complete job duties. Professional Designation/Certification (one of the following are required): Registration, in good standing, with a regulating professional body Ontario College of Social Work and Social Service Worker (e.g. OCSWSSW); or Certification, in good standing, with a professional certifying organization within health or social service fields e.g. Certified Psychosocial Rehabilitation and Recovery Practitioner (CPRRP) from Psychosocial Rehabilitation (PSR Canada); or Working towards obtaining one of the above within 6 months of hire. Organization must have a written code of ethics, core values and standards that are consistent with those of CMHAS/M. Certification must require members to meet a core set of competencies, have a requirement for continuous learning to maintain certification and be approved by CMHAS/M. Hourly, Step 1: $28.12 - Step 4: $33.09 College Diploma Hourly, Step 1: $25.37 - Step 4: $29.84 *Successful candidates for this position will start at Step 1 of the salary grid.
