We work collaboratively with healthcare professionals, community partners, and patients to foster innovation, excellence, and transformation in care delivery meeting the evolving needs of the diverse communities we serve.Reporting to the Executive Director, the Project Manager will be responsible for supporting high-value strategic initiatives; supporting the development, implementation, and management of projects with cross-functional teams to achieve the organizations strategic goals. The primary focus of the role will be to support healthcare integration initiatives that contribute to the continuum of connected healthcare services and improve patient experience through efforts identified across health and social service organizations.Duties and Responsibilities:Contributes to the organizations efforts and initiatives to enhance HFHT mission, vision, and values.Makes recommendations on services, policies and programs while striving to continuously improve processes.Ensures consistency through the implementation of consistent tools and methodologies while maintaining appropriate documentation which clearly illustrates project progress and success at completion.Prepares, reviews, and submits reports to department staff, SLT, the Board, and external agencies that are clear, concise, transparent, and accurate.Ensures that patient experience and improving patient care remains central, building equitable, efficient, and quality healthcare services.Supports the broader HFHT healthcare integration initiatives, while also balancing consistency and standardization across, and unique needs of, the designated geographical neighbourhoods.Provides project management support/expertise across the organization, as needed.Project ConsultingFacilitates and leads in supporting strategic initiatives.Develops project plans for projects, evaluation, implementation, and communication.Application of planning methodologies for targeted rapid cycle improvements (A3, PDSA).Proactively takes appropriate action to keep the project on track by identifying risks/barriers to success, scope creep, schedule challenges, and offering mitigation strategies.Chairs/participates in relevant working groups for organizational projects and initiatives.Process ImprovementActs as a resource for expertise, facilitation, education, and application of process improvement tools including:Lean Six Sigma methodologies and toolsProblem identification and root cause analysisBrainstorming and focus group facilitationChange management toolsStatistical analysis including run and control chartsPerformance MeasurementProvides consultation/guidance regarding appropriate indicators, targets, and data sources to measure performance of projects.Prepares and presents reports that analyze trends.Interprets data related to projects to inform decisions of stakeholders, including Senior Leadership.QualificationsUndergraduate degree in a relevant health related field required, masters degree preferred.Regulated Health Professional and a member in good standing with the relevant professional college or association (Social Work, Psychology, Nursing, OT, PT, etc.) is considered an asset.3-5 years of practical related experience in a primary care or community setting is considered an asset.Project Management Professional (PMP) or Lean Six Sigma black belt designation or equivalent experience.Excellent written and oral communication skills including a high-level of comfort with medical jargon and language.Excellent time management and prioritization skills.Competency in project management, process improvement, business planning, performance measurement and evaluation.Experience managing complex projects throughout the project lifecycle with demonstrated analytical, problem solving, critical thinking, conflict resolution, and negotiation skills.Strong team, relationship and consensus building skills.Ability to effectively collaborate with physicians, senior leadership, all members of the healthcare team and external stakeholders.Understands appropriate level of detail required for a given situation and can adjust/move from brief executive level to deep detail with ease.Why join the HFHT?Competitive Employee Value Proposition including, but not limited to:Healthcare of Ontario Pension (HOOPP)Extended health care benefits including health, dental, vision & critical Illness insuranceMeaningful, purpose-based work12 paid Stat holidays and one (1) extra float dayHybrid work scheduleOngoing green initiativesSummaryClassification: Non-unionSchedule: Monday-FridayEmployee Class: Full Time (1.0 FTE)Primary Location: 123 James Street North, Suite 200, Hamilton, ONDate Available: ASAPSalary: $84,500 $96,536.72Application Instructions:Interested applicants please submit rsum and cover letter as one document using naming convention Last name, First name_Position by email: Note:If successful in receiving a job offer with the Hamilton Family Health Team, new hires may be required to provide proof of full COVID-19 vaccination prior to start date as a condition of their employment.
