Lead Access Transition Services & Waitlist Jasper Alberta Management Assisted Living Alberta is seeking a dynamic and results-driven individual to lead and help ensure provincial Access, Transition Services & Waitlist Management programs meet the highest standards of quality, safety, efficiency, and patient satisfaction. The Lead, Access, Transition Services and Waitlist Management (Lead) will coordinate improved access to continuing care services, apply the Aging-in-the-Right-Place philosophy, and support safe and efficient transitions as clients care needs evolve. Reporting directly to the Senior Program Officer, ALC Transition & Integration, the Lead is responsible for overseeing and optimizing Albertas centralized access, transition, and waitlist services. The Lead will provide strategic oversight, operational guidance, and continuous improvement leadership, ensuring safe, high-quality continuing care access and transition services. Key responsibilities include: - Providing operational leadership and system-level care coordination expertise to on-the-ground delivery teams across Albertas regional corridors. - Overseeing and optimizing Albertas centralized access, transition, and waitlist services. - Ensuring smooth transitions from both acute care and community settings, facilitating operational alignment, and ensuring consistency across all of ALAs diverse zones/corridors. Requirements: - A bachelors degree in nursing, health administration, social work, or a related field. - A minimum of 10 years of progressive leadership experience in healthcare or social services operations. - Strong knowledge of Alberta/Canadas acute care, continuing care, and community-based care systems. Preferred qualifications: - Masters degree preferred. - Certification in Lean, Six Sigma, or other process improvement methodologies. - Certification in project management, healthcare leadership, or a related discipline is an asset.
