Reporting to the Manager Northwest Regional Renal Program theRegional Chronic Kidney Care Lead will play a critical role in engaging Indigenous partners, buildingrelationships, and leading renal related projects to improve wellbeing, enable culturally safe kidney care and reducethe impact of chronic kidney disease on Indigenous people. The Regional Chronic Kidney Care Lead will establish and provide support for the care and management of Home dialysis (peritoneal dialysis and home hemodialysis) for renalpatients within the Northwest Region. The Lead will provide Indigenous Communities with access to knowledge on ChronicKidney Disease and home therapy options. The Lead will work with leadership at the Ontario Renal Networkand Indigenous Renal Clinical Programs to collaborate and provide renal specific knowledge and tools tosupport current and future renal patients interested in a home therapy. The Lead will be responsible to plan, implement and evaluate needsbasedcontinuing education for Indigenous Communities and health professionals in collaboration with RenalLeadership and other key stakeholders. The Lead will utilize their extensive knowledge base and experience tosupport health professionals in decisionmaking and applying knowledge into practice. They will assist in skilldevelopment, including clinical, problem solving and critical thinking. The Lead will be required to travel throughoutNorthwestern Ontario to various Indigenous Communities. Employment Requirements Education/Experience - Baccalaureate degree in a Regulated Healthcare Profession - Minimum 5 years clinical renal experience - 3 years clinical leadership experience is preferred - Unencumbered registration with applicable regulatory body Skills/Abilities - Demonstrated leadership and teamwork skills to support, coach, direct, mentor and teach activities to Clinical staff and to promote positive staff relations - Demonstrated strong communication skills both oral and written - Demonstrated strong interpersonal skills and emotionalintelligence to interact effectively with staff, patients, family supports and community partners - Demonstrated ability to work effectively individually and withina team setting - Awareness of Indigenous culture and protocols specific to theIndigenous population - Demonstrated high level of selfinitiative and direction - Proficient in the use of computer applications such as emailsystems, Microsoft Office, PowerPoint and Excel - Demonstrated commitment to the principles of Patient &Family Centered Care - Demonstrated commitment to professional practice andcontribution to professional Workplace/environment Assets - Knowledge of local, regional and provincial issues impactingIndigenous communities, Indigenous language spoken and written Salary Range $49.500 to 61.870 The Health Sciences Centre strives to ensure the safety andsecurity of the patients, visitors, employees and assets financial and otherwise.
