From our two locations in Belleville and Trenton, BQWCHC offers programs and services to improve individual health and community wellbeing, focused on health equity. Responsibilities - Perform assessments, diagnosis, treatments, health education/counselling, and other medical functions for primary care clients. - Conduct sameday access appointments, home visits as required and participate in the Centres oncall services on rotation to provide afterhours services. - Use an equity and clientcentered approach, engaging clients and collaborating with other services mindful of social determinants of health. - Participate in continuous quality improvement activities. - Participate in case conferences, make referrals to interdisciplinary teams and external resources, and correspond with a collaborative team. - Contribute to the development, implementation, and evaluation of clinical protocols, medical directives, and other process improvements. - Support and evaluate students, medical residents, and other learners. Qualifications - Registered Nurse (RN) licence to practice from the College of Nurses of Ontario. - Experience as a Nurse Practitioner, preferably in primary health care or a community setting. - Experience working effectively with communities and individuals who face barriers to accessing health care and supports, with a focus on mental health challenges and problematic substance use. - Demonstrated problemsolving, organisational, and interpersonal skills, with the ability to work effectively in a collaborative, interdisciplinary team environment. - Understanding of and experience utilizing a traumainformed approach. - Work hours include one evening shift per week and participation in an oncall rotation. #J-18808-Ljbffr
