What to expect in the role This is a leadership role overseeing a team of frontline Community Disability Practitioners in our Group Living department. Your primary responsibility will be supporting your staff teams. You will do the scheduling, training and coaching of your team members, and manage their performance. In addition to working with your team, you will also manage the caseload of the clients in our Group Living program, ensuring all documentation, ISPs, Planned Approaches, and Behavioural Support Plans are completed and collaborating with their families, guardians, physicians, and other professionals to provide the highest quality of service. This team supports individuals with a variety of diagnoses including but not limited to Dementia, Medical Needs Down Syndrome, Autism, Anxiety, Depression and Developmental disabilities. The individuals all have individualized approach plans that you would be required to be knowledgeable with. In this role you will also be responsible for corresponding with guardians, following PDD requirements and submissions, as well as ensuring your team is compliant with all training requirements. This position may require working on site, weekend availability, driving clients, personal care, household cleaning, and also could require physically demanding tasks related to client needs and or behaviours. Flexibility is required as you will be part of the oncall rotation. Lead and assess the research, development and implementation of programs to meet individuals needs and goals. Ensure that individuals fulfill their goals, objectives and aspirations includingmeeting the individuals multifaceted and challenging behavioural and medical needs through the direct service staff Review client goals with frontline staff and provide feedback to Manager Ensure the staffs appropriate involvement with the individual in all areas Liaise with guardians, family, clients network, and other community support services and resources Attend, participate and lead network meetings as required, insuring direct staff involvement as required by P&P Ensure regular staff supervision and annual performance evaluations Familiarity of funding mechanisms Familiarity of legislation related to agency operations and a good understanding of Employment and Labour Standards Weekend and evening flexibility required Oncall as per rotation will be an expectation Set Schedule & Rate of Pay This position supervises a team of practitioners in a group living setting to support clients 24/7. The schedule for this two-year temporary position is 40 hours a week and will end with the possibility of an extension. This position will require availability for weekend oncall duty as per Team Lead rotation. The salary is starting from $29.95/ hour and will qualify for benefits after a three month probationary period. Minimum of a Diploma in Human Services studies from a recognized institution. 3-5 years related work experience in the Human Services field. 2-3 years of supervisory experience Medium to high level computer literacy and strong reporting skills. A positive attitude that acknowledges the gifts, talents, and passion of persons with disabilities and their support staff, an aptitude for service & support and a relevant knowledge and skill base. Demonstrate the ability to establish an atmosphere of trust, openness and communication and an attitude based on working together and shared objectives Current mandatory training (First Aid & CPR, Nonviolent Crisis Intervention, Positive Behaviour Supports, Abuse Prevention & Response Protocol, Med Admin) Valid class 5 driver license and a reliable vehicle with minimum $2,000,000 liability insurance by time of hire Comfortable leading and supporting staff through trainings, supervisions, interpersonal conflicts, and disciplinary concerns when needed Preferred Requirements Previous Advisor experience, or leading a team Proven experience of managing time well Demonstrated ability to complete documentation thoroughly and accurately Experience supporting a variety of diagnoses Knowledge on community supports & resources available #J-18808-Ljbffr
