Position Summary We are seeking a highly motivated and experienced Registered Nurse to join our Renal Transplant Clinic as a Primary Care Nurse. The Registered Nurse (RN) is a regulated health care professional who adheres to and models the Standards and Guidelines of the College of Nurses of Ontario; the SJHH mission, vision and values, policies and procedures; and applicable specialty standards. Responsibilities Coordinating the care of patients from the time of referral throughout the transplant journey (initial assessment, listing, follow-up, donation/transplantation, post-transplant/donation care). Providing patient education and supporting the patients'' understanding of the transplant journey. Acting as the key contact person for patients/families, referring physicians, and other healthcare providers both internally and externally. Collaborating and participating in initiatives locally and with partner organizations such as Trillium Gift of Life Network, Canadian Blood Services, and the Ontario Renal Network. Qualifications Current registration with the College of Nurses of Ontario required. Minimum of two (2) years of nephrology, dialysis, or transplant nursing experience required. Demonstrated recent leadership experience in renal or related nursing required, e.g. experience in Charge Nurse and/or Team Lead roles. Completion of additional nephrology or transplantrelated education (e.g., courses/certificate, cNeph). Active participation in, or leadership of, committees. Delivery of nephrology or transplantrelated educational presentations. Current BCLS/HCP certification required. Minimum of one (1) year of transplant experience preferred. Canadian Nurses Association certification in Nephrology cNeph(C) preferred. Membership with RNAO or another relevant professional association preferred. Current CPI and/or GPA training preferred. Excellent attendance record required. A resume outlining experience, further education, and any special courses required and must accompany the application. Demonstrated Knowledge, Skills And Abilities Advanced case management, organization, time management, and coordination skills with the ability to prioritize workload to meet urgent and rapidly changing timelines. Selfdirected with strong leadership, problem solving, decision making, critical thinking, team building and interpersonal skills. Effective communication and education with patients and families. Collaborative practice and clear communication with members of the interdisciplinary team. Proficiency with computers (Microsoft Office, internet browser searches, data entry and patient information systems). Commitment to ongoing learning, professional development, and continuous quality improvement.
