Working both in-clinic and through community outreach, the clinician provides comprehensive assessment, consultation, counselling, and short-term management for complex older adults. This role works in close collaboration with the clinics Geriatricians and Nurse Practitioners and develops strong partnerships with community providers to support coordinated, patient- and family-centred care.**Qualifications*** Registered and in good standing with the respective regulatory body: + College of Occupational Therapists of Ontario (COTO) **or** + College of Physiotherapists of Ontario* Masters degree in Occupational Therapy or Physical Therapy (or equivalent) from a recognized university* Minimum 23 years experience working with the geriatric population* BCLS certification required, or willingness to obtain* MOCA certification required, or willingness to obtain within 3 months of hire* Valid Ontario Drivers License, access to a vehicle, and a clean driving abstract**Knowledge, Skills & Abilities*** Excellence in interpersonal communication and team building, fostering positive collaboration with internal and external partners* Advanced assessment and management skills across physical and cognitive domains* Working knowledge of: + Geriatric syndromes and geriatric psychiatry + Community resources and crisis intervention + Regulated Health Professions Act, Mental Health Act, Health Care Consent Act, and Substitute Decisions Act* Demonstrated commitment to evidence-based, patient- and family-inspired care* Superior verbal and written communication skills* Strong critical thinking, problem-solving, and clinical judgment* Ability to work effectively both independently and within an interprofessional team* Commitment to clinical innovation and lifelong learning* Commitment to the principles of diversity, equity, and inclusion* Proven reliability in attendance in accordance with organizational policy* Models behaviour aligned with the values of Southlake HealthSouthlake Health is fully committed to a culture of belonging and an inclusive environment that encourages every team member to lead within their role, generate innovative ideas that reinforce our mission and goal to create a healthy workplace where our community can thrive.Regular and reliable attendance is an expected requirement of this position.We are committed to a selection process and work environment that is inclusive and barrier-free.
