Job Details Job Number: J0126-0419 Job Title: Team Lead - Registered Nurse Job Type: Part Time Location: Alliston Category: Nursing Number of Open Positions: 2 Department: Medicine Union: ONA Salary: $40.24 - $57.68/Hour FTE: 0.5 Job Summary The Team Leader works collaboratively with the Manager and the Medicine/Surgical teamincluding physicians, nurses, allied health professionals, and support staffto provide leadership that reflects the organizations vision, mission, values, and strategic goals. The Team Leader is responsible for coordinating patient care and overseeing all unit activities, ensuring that every patient admitted to Med Surg receives the highest quality of care. As an active member of the programs leadership team, the Team Leader participates in staff and team meetings, as well as unit and hospitalwide initiatives (e.g., Bedside Shift Reporting). Working closely with the Manager, the Team Leader helps ensure appropriate staffing levels, skill mix, and delegation of daily operations while maintaining a focus on delivering safe, patientcentered, highquality care. The Team Leader fosters collaboration among patients, families, staff, physicians, and external partners to promote evidence-based practices, effective patient flow, and a positive care experience. Responsibilities Attends and/or leads daily team huddles independently and in collaboration with the Manager, including discussion of hospital and unit initiatives, performance data, and dialogue with invited guests. Ensures compliance with unit/hospital initiatives through participation and observation (e.g., Bedside shifttoshift report, Hand Hygiene). Coordinates the flow of department personnel, demonstrating flexibility and creativity to ensure timely, safe care to patients. Actively participates in resolving Risk Management File (RL6) submissions and provides Manager with a report. Participates in evaluating and planning interventions to decrease the risk of future incidents or near misses. Assumes the role of Control Person in all Emergency Codes, providing clinical expertise. Participates in maintaining statistical data as required by the program. Fosters an environment that enables communication, creative thinking, and participation by modeling approachability and visibility. Provides mentoring, guidance, and acts as a resource and role model to both novice and experienced nurses within the unit on issues related to patient flow and CNO professional standards. Monitors and ensures compliance with clinical standards as well as unitbased policies and procedures and clinical practice guidelines. Works with leadership to implement and sustain process improvement initiatives. Ensures staff and patient safety through knowledge of Code policies and procedures. Participates in case reviews to support staff development and improve patient care. Develops daily nurse/patient assignments ensuring that the assignment reflects RN or RPN scope of practice, experience, and knowledge of the type of care required for the unit. Fills daily staffing needs in collaboration with Clerk, Staffing Office, and Program Manager as required. Reevaluates Enhanced Observation needs in conjunction with team and Primary Care RN on a shiftbyshift basis. Adjusts nurse/patient assignments as needed to ensure safety, balance, and the minimization of overtime. Ensures breaks are planned around actual or anticipated patient activity, ensuring a safe working environment and adherence to the collective agreement. Coordinates patient flow in collaboration with the ADT nurse by facilitating the timely admission, transfer, and discharge of patients. Coordinates admissions, transfers in, transfers out, and discharges with the ADT nurse, ER team leader, sending facilities, and receiving facilities. Determines Expected Date of Discharge (EDD) for new admissions and transfers, and places EDD in the EMR and on the patient whiteboard. Coordinates multidisciplinary team to identify barriers to discharge and act upon them to provide resolution prior to EDD. Coordinates and leads daily bullet rounds (ensuring compliance with a goal of 30minute timeframe). Ensures all quality initiatives and safety checks are completed as needed (daily, weekly, monthly). Communicates and collaborates with the Clinical Manager. Assists primary nurse in communication with MRP and specialists in their absence, ensuring patient updates are received from each nurse. Participates as a member of the Medicine Quality Committee, Staff Meetings and other committees to disseminate messages from these committees to the team. Provides Manager updates on patient care issues including family, substitute decision maker, POA, etc. Notifies Manager of potential complaints. Informs Manager of staffing concerns. Makes Manager aware of supply and equipment issues. Informs Manager of trends affecting patient flow. Provides initial investigation of risk issues to Manager. Actively involved in Medicine Quality and Senior Friendly initiatives. Qualifications and Experience Registered and in good standing with the College of Nurses (CNO) as a Registered Nurse. Current Basic Cardiac Life Support (BCLS) certificate. Current Advanced Cardiac Life Support (ACLS). CC1 required; CC2 preferred. Management of Aggressive Behaviour or equivalent preferred. Certificate in Gentle Persuasive Approach preferred. Five (5) years of current acute/cardiac care nursing experience. Active participation in Medicine Quality Committee and/or other leadership teams is considered an asset. Proficient in Meditech and Carescape Monitoring System. Additional Leadership, Management, Conflict Resolution courses would be an asset. Indepth knowledge of LEAN methodology Green Belt certification preferred. Skills and Abilities IV therapy initiation and maintenance (parenteral therapy). Excellent assessment, communication, organization, problemsolving, decisionmaking, and criticalthinking skills. Initiative and ability to function both independently and as an interprofessional team member in a fastpaced, demanding environment. Strong abilities to facilitate patient and familycentered care and establish therapeutic relationships. Understanding and compliance with patient and staff safety policies and procedures that foster a safe and inclusive environment for all. Hours of Work This position requires flexibility to work all shifts, including weekends, and both 8 and 12hour tours, as determined by unit needs. The Team Leader must also be prepared to assume a full patient assignment and provide direct patient care as required.
