Shape impactful community services as a Community Programs Coordinator with Hospice Niagara. This role focuses on client assessments, volunteer oversight, and program evaluations to enhance wellness. Reporting directly to the Director of Community Programs, you will assess client suitability for diverse community service initiatives while managing program budgets and supervising volunteers. This position is essential for fostering client wellness and caregiver support in the Niagara area. Your ability to evaluate program effectiveness will drive continuous improvements and ensure clients receive the care they need. Key Responsibilities: Conduct intake and assessment for clients and caregivers Supervise volunteers and performance evaluations Maintain an accurate client information database Coordinate and manage weekly programs at various locations Develop and evaluate program outcomes and reports Requirements: Graduate of an RPN or related program Experience in palliative care settings preferred Excellent communication and organizational skills Strong active listening and problem-solving abilities Valid drivers license and access to a vehicle Leverage your interpersonal skills and compassion to drive community wellness and caregiving at Hospice Niagara. #J-18808-Ljbffr
