Seeking a dedicated Registered Practical Nurse for a community-focused dual diagnosis rehabilitation program. This position requires a passionate nurse to join the Forensic Dual Diagnosis Transitional Housing Rehabilitation Program, where you''ll be essential in delivering recovery-oriented support for individuals living with dual diagnoses. You''ll coordinate with an inter-professional team to manage referrals, oversee risk management, and facilitate community resources. Your involvement will ensure that clients receive tailored interventions aligned with their transition to community living. Key Responsibilities: Coordinate community mental health services for dual diagnosis clients Monitor and manage behavioral support plans Engage actively in case conferences and assessments Provide education and support to community partners Maintain compliance with mental health standards and legislation Requirements: Good standing with the Ontario Nursing College Current CPR certification for Health Care Providers Knowledge of forensic mental health practices Effective collaboration within a multidisciplinary team Recent experience in psychiatric nursing preferred Leverage your skills to assist clients on their journey towards recovery within a supportive community setting. #J-18808-Ljbffr
