Position Title: Case Manager, Dual Diagnosis/Justice Classification: C Reports To: Program Supervisor Position Status: Temporary Full Time Salary: $32.17-39.30/hour, per OPSEU/SEFPO Local 684 Collective Agreement POSITION SUMMARY The Case Manager (Dual Diagnosis / Justice Program) is a member of a team of professionals who deliver justice services to individuals experiencing mental illness, including concurrent disorders and/or an intellectual disability, and who come into conflict with the law, or are at risk of coming into conflict with the law. This Case Manager carries out these responsibilities in accordance with the mission, vision, and values of CMHA, relevant legislation and regulations and organizational policies, to promote the delivery of efficient and high-quality services. KEY RESPONSIBILITIES Provides Justice Services: Service Engagement, Assessment, Service Delivery, Pre-Charge Diversion, Court Support, Transitional Case Management, Release from Custody, Dual Diagnosis, Community Relations and Advocacy, and Organizational Responsibility Delivers case management services for individuals with serious mental health conditions and/or substance use disorders as well as their significant others. Specific areas of support may include the following based on individual needs: housing/home maintenance, food security, self-care, physical health, harm reduction, diagnosis and treatment, financial, transportation, legal, and varying daytime activities. Exhibits exemplary professional conduct. Engages in effective internal and external relationships with all staff, clients and volunteers of the organization, other organizations, and the community. Promotes and supports a safe work environment. Improve quality and enhance services. Expand services by investing in priority areas. Fill gaps in care through innovative solutions. Improve access through a new provincial program and approach to navigation. QUALIFICATIONS Education, Certification & Experience A university Bachelors degree in a Human Services related field, preferably Social Work and/or Criminology Current registration with OCSWSSW or a regulated body under the Regulated Health Professions Act, preferred Demonstrated knowledge of community resources and organizations Certification in First Aid/CPR, mandatory Certification in ASIST (or willing to obtain on own within 3 months) Minimum three years experience in the fields of mental health and addictions with an emphasis in community-based services Knowledge and experience in conducting bio-psychosocial assessments Experience in the criminal justice system and knowledge of community resources Knowledge, Skills and Abilities Knowledge and experience in conducting bio-psychosocial assessments. Experience in the criminal justice system and knowledge of community resources is an asset. Ability to work in partnership with clients. Extensive knowledge and understanding of the mental health and addictions services. Extensive knowledge of standardized screening and assessment tools. Extensive knowledge of community services supports and resources. Ability to apply Recovery principles and empowerment-orientated philosophies and practices in work with clients. Demonstrated ability to observe boundaries, engage in appropriate emotional regulation, refrain from dual relationship with clients. Ability to demonstrate diplomacy and negotiation skills in client and system advocacy. Ability to make sound clinical judgments and to effectively communicate that to others. Other Acceptable Criminal Record Check and Vulnerable Sector Check upon hire and to be maintained throughout employment. Ability to provide effective client services in English and French is preferred. Able to meet the essential physical requirements of the position including ability to sit for prolonged periods. Valid G drivers license, proof of insurance and access to a reliable vehicle.
