Elevate patient care as an Integrated Care Dietitian with CBI Home Health in the North York region. Your role will focus on providing critical nutrition guidance for patients transitioning back home from hospital stays. As a Registered Dietitian, you''ll be instrumental in the Integrated Care Program. Your duties will include working with an interdisciplinary team to ensure comprehensive support for each patient''s unique dietary needs. Your commitment to person and family-centered care will help empower clients and improve their overall well-being. Key Responsibilities: Assist patients in transitioning home with nutrition support Collaborate with healthcare professionals in an interdisciplinary setting Develop plans that prevent readmissions and promote health Conduct thorough assessments and offer personalized dietary advice Monitor and adapt nutrition plans based on patient feedback Requirements: Currently registered with the College of Dietitians of Ontario Experience with tube feeding is beneficial Ability to communicate effectively with patients and teams Travel within the community as required Bilingual proficiency in English and French is valued Support patients to thrive at home using your nutrition expertise with CBI Home Health. #J-18808-Ljbffr
