Pay Per Visit Competitive rate paid per client visit rather than care code or care time. Additional pay for visits exceeding 1 hour 59 minutes. Model allows flexibility while completing daily workload. Requires professional judgment in client care. Fulltime nurses guaranteed 70 visits biweekly for a predictable income. Responsibilities Conduct comprehensive client assessments, develop care plans, implement and evaluate care based on evidencebased practices and the clients unique needs. Serve as the primary provider for a client caseload, acting as pointofcare lead while collaborating with other care team members and support functions. Assist clients through care transitions by connecting them with community support services. Requirements Current registration with the College of Nurses of Ontario. Canadian Nurses Association (CNA) specialty certification is an asset. Enthusiasm for community care and a passion for evidenceinformed practice and excellence. Current CPR certificate required. Valid drivers licence and vehicle (travel required). Updated immunisation records, a clear criminal background check, and a vulnerable sector check required upon hire. Willingness to attain required Saint Elizabeth certifications. Availability to work weekdays and every other weekend. Physical capacity to lift up to 55 pounds, walk, climb stairs, stand, bend, reach, and move frequently. Pay The hiring pay range is $27.30$30.30 per visit, based on experience.
