Lead a dedicated team in home care at Bayshore HealthCare as a Clinical Leader in Toronto, Ontario. Guide personalized care plans and support clients wishing to age in place. As a Clinical Leader for Bayshore HealthCare, you will oversee a local Neighborhood Health Team dedicated to providing in-home care services. In this hands-on role, you'll design care strategies using a holistic approach tailored to each clients needs. Collaborating with doctors and community health providers, you are vital in addressing the changing challenges clients face, ensuring they receive exceptional care. Key Responsibilities: Develop comprehensive care plans for clients and families Direct and monitor in-home services for field staff Hire, train, and supervise healthcare team performance Manage care planning with clients and their families Adapt services to cater to evolving client health needs Requirements: Current RN, OT, PT, or RSW license in Ontario Five years of healthcare experience in the community Two years in a leadership role with management duties Valid CPR certification and drivers license Strong preference for experience with geriatric populations Utilize your leadership skills and healthcare expertise to enhance in-home care at Bayshore HealthCare. #J-18808-Ljbffr
