Job DescriptionInsight Global is seeking a Behaviour Support Worker to join a multidisciplinary Behaviour Support Worker (BSW) team supporting older adults (65+) with responsive behaviours across UHN sites. This is a handson, frontline role focused on behavioural assessment, implementation of care strategies, and supporting successful patient transitions through acute care settings. This is a newly created position due to increased funding and referral volumes.Key Responsibilities/ Day to day work- Provide handson behavioural support to older adult patients with responsive behaviours- Work across multiple UHN sites including Toronto Western Hospital, Toronto - Grace, West Park, and in collaboration with CAMH- Conduct behavioural observations and collect information from patients, caregivers, and care teams- Collaborate with mental health clinicians to contribute to and implement behavioural care plans- Support care teams in managing behaviour and preparing patients for next steps in their care journey- Provide education and informal coaching to interprofessional care teams- Support transitions to LongTerm Care Homes when appropriate- Maintain accurate clinical and operational documentation in accordance with hospital standardsRequired QualificationsCompletion of a 2year college diploma in Behavioural Sciences, Mental Health Studies, Social Service Worker, Gerontology, or a related fieldExperience with older adults/geriatrics populationsAt least 1 year of experience supporting patients in acute care, mental health, or longterm care settingsStrong interpersonal, communication, and timemanagement skillsAbility to work independently across multiple sites and manage competing prioritiesComfort working in fastpaced, complex clinical environmentsFirst Aid and CPR certificationPrevious experience as a PSW or similar frontline care roleBehavioural training or certifications such as P.I.E.C.E.S., GPA, UFIRST, or DementiAbilityNicetoHave Qualifications- Behavioural Science Technician background- Valid drivers license and access to a reliable vehicleCompensation:$27-$34/hExact compensation may vary based on several factors, including skills, experience, and education.We may use artificial intelligence tools to assist with the screening, assessment, or selection of potential applicants for this position.
