Our Home Care Solutions division supports the Greater Hamilton and Burlington areas, enhancing the quality of life, dignity, and independence of clients who remain in the comfort of their own homes. Location: Home to home in the Greater Hamilton and Burlington area. Driving within the community is required. Pay: Pay per visit. Shifts: Monday through Friday 3:00pm to 11:00pm as assigned. Statutory holiday rotation. Oncall rotation. What Youll Do Participate in oncall rotations as required (typically twice per month overnight). Conduct inhome assessments and deliver nursing care based on individualized care plans. Administer medications, wound care, IV therapy, palliative care, pediatric care, and other clinical interventions. Monitor patient progress and update care plans accordingly. Educate patients and caregivers on disease management, medication adherence, and health promotion. Collaborate with interdisciplinary teams and delegate tasks to ensure continuity of care. Safe and Supportive Environment: adherence to strict health and safety protocols. Education & Certification Graduate of an accredited School of Nursing (RPN). Current registration in good standing with the provincial regulatory body (CNO). LEAP training or palliative certification considered an asset. Experience 25 years of recent relevant nursing experience in a fastpaced clinic or medical environment. Experience with wound care, diabetes management, trach/vent, or IV care is an asset. Other Requirements Valid G2 or G drivers license and access to a reliable vehicle. Willingness to travel regularly within the Hamilton/Burlington area in all weather conditions.
