The Director, Primary Care Transformation and Academic Partnerships will provide strategic and operational leadership to advance SCHCs Neighbourhood Health Model, strengthen relationships with community physicians, and foster sustainable partnerships with universities, colleges, and training institutions. This role is responsible for advancing partnership strategies that improve access, attachment, continuity of care, workforce development, teaching and learning opportunities, and innovation in community-based health care. Working closely with internal leaders, community physicians, academic partners, and system partners, the Director will help position SCHC as a leading community-based academic primary care organization. The Director is a key member of the leadership team and will support the successful implementation of strategic priorities aligned with Ontario Health, Scarborough Ontario Health Team, and SCHCs mission, vision, and values.Key Areas of Responsibility:Primary Care Transformation & System IntegrationLead implementation of strategic initiatives that advance SCHCs Neighbourhood Health Model and integrated primary care strategy. Strengthen pathways that connect unattached residents and community providers to SCHC programs and services. Support development of innovative, team-based models of care that improve access, attachment, continuity, and health outcomes. Identify opportunities for spread, scale, and sustainability of successful models across Scarborough. Monitor provincial policy, funding, and system trends related to primary care reform and advise the Vice President on opportunities and risks. Physician Partnerships & Community Provider EngagementSteward relationships with community physicians to support shared care and integrated service delivery. Build practical referral, collaboration, and communication pathways between independent providers and SCHC teams. Identify barriers to provider participation and implement solutions that strengthen engagement. Monitor partnership agreements, shared initiatives, and collaborative models of care. Academic Partnerships & Learning Health System DevelopmentSupport the development and on-going sustainment of partnerships with universities, colleges, and training institutions to expand learner placements, teaching opportunities, and joint initiatives. Work with internal clinical and operational leaders to create high-quality learning environments for learners.Support building capacity for individuals across the organization to advance their roles as faculty members. Advance SCHCs academic profile through partnerships in education, research, evaluation, and knowledge exchange. Promote a culture of learning, innovation, and evidence-informed practice across teams. Strategy, Planning & PerformanceDevelop annual workplans, deliverables, metrics, and reporting frameworks for the portfolio. Use data and stakeholder feedback to evaluate impact, improve performance, and guide decision-making. Prepare reports, presentations, briefing materials, and recommendations for senior leadership and partners. Support business cases, funding proposals, and growth opportunities that advance strategic priorities. Ensure compliance with funder expectations, organizational policies, and accountability requirements. People Leadership & CollaborationProvide leadership, coaching, and oversight to assigned staff and/or project teams. Foster a collaborative, inclusive, and high-performing team culture grounded in SCHC values. Work closely with internal leaders across primary care, community health, innovation, professional practice, and operations to align initiatives and resources. Lead through influence in matrixed and cross-functional environments. Educational and/or Professional Qualifications:Masters degree in Health Administration, Public Health, Health Sciences, Business, Education, or related field preferred. Bachelors degree with equivalent senior experience will be considered. Minimum 57 years progressive leadership experience in healthcare, primary care, community health, system integration, academic partnerships, or related sectors. Strong knowledge of Ontarios healthcare system, primary care landscape, and health equity approaches. Experience building partnerships with physicians, academic institutions, or external stakeholders. Demonstrated success leading complex initiatives with multiple partners. Strong project management, facilitation, and organizational skills. Excellent written, verbal, and interpersonal communication skills. Strong analytical skills and comfort using data to guide decisions. Level of Experience:Experience in a Community Health Centre, Family Health Team, Ontario Health Team, hospital, or community-based setting. Experience with student placement programs, teaching environments, or academic affiliations. Experience in strategy implementation, service redesign, or change management. Experience securing grants, partnerships, or new program opportunities.
