Location:Remote (7+ hr overlap with ET/PT) Engagement:Contract-to-hire (30–45-day build → full-time option) Comp:$1500USD base wage + 1% equityAbout Us Apex Dental Systems builds AI-driven patient acquisition and receptionist solutions for dental and orthodontic practices. We’re launching a24/7 Voice AI receptionistthat achievesfeature parity with leading incumbents(functionality only—no code/IP reuse) and adds orthodontic-specific workflows.What You’ll Build (Feature Parity Targets) 24/7 inbound call handling(after-hours + overflow), unlimited minutes. Appointment scheduling & reschedulinginto the practice’s PMS (Open Dental, Dentrix, EagleSoft, Cloud 9, etc.) viaNexHealth(preferred) orKeragon / direct APIswhen feasible. Provider/appointment-type mappingconfigurable per practice Language support:English (phase 1) + Spanish (phase 2); later add others. Custom screening questionsby practice; capture insurance and referral source. Booking confirmation & follow-upvia SMS/email; no-show/recall nudges. Call recording + AI analytics(intent resolution, containment, CSAT proxy). Phone system integration:Twilio Voice/SIP; caller ID and routing rules. Admin portal:Multi-tenant controls, prompts, hours, holidays, provider availability, and reporting. Security & compliance:HIPAA, PHI redaction, audit logs, role-based access, encryption at rest/in transit.Core Stack (preferred) Voice & Orchestration:Retell AI(call flow, LLM orchestration, tools),Twilio(Voice, SIP, Studio if needed). Scheduling/EHR:NexHealth API(primary),Keragonconnectors where appropriate. Backend:Node.js/TypeScript or Python (FastAPI), REST/GraphQL, webhooks. Ops:CI/CD (GitHub Actions), error monitoring, alerting (PagerDuty/Slack).Responsibilities Own theend-to-end Retell AI bot : call flows, prompt/tool design, function calling, and latency optimization. ImplementTwilio telephony(numbers, IVR, overflow, failover, voicemail). Buildscheduling integrations(NexHealth first; fallbacks where coverage allows). Designmulti-tenantconfiguration: providers, appointment types, hours, screening questions, languages, consent scripts. DeliverHIPAA-readyarchitecture: data minimization, PHI redaction, audit trails, BAA alignment, least-privilege IAM. Stand upadmin portalfor non-technical staff to manage practices and see analytics. Instrumentquality metrics : first-call resolution/containment, booking rate, show rate, average handle time, interruption rate, latency. Build asafe handoffto human staff on edge cases (warm transfer, ticket drop). Document everythingGovernance & Access Requirements Code in ourGitHubunder our org; we own all infra accounts. Infrastructure-as-Code(Terraform) for reproducible environments. Separateprod/stageaccounts; no personal credentials. RBACfor Retell, Twilio, NexHealth, Keragon; keys in Secrets Manager. Automated backups,runbooks , and offboarding plan to prevent vendor lock-in.15/30/45-Day Deliverables Day 15 (MVP ready for internal use): Retell bot handling inbound calls with basic intent set (new patient, existing patient, billing, emergency, hours). Twilio numbers provisioned; recordings + redaction; Slack alerting on fails. NexHealth sandbox connected;book + reschedule2 appointment types; English live. Basic admin panel (practice setup, provider/appointment mapping, hours). Day 30 (Pilot with 3–5 practices): Multi-tenant support;up to 3 providers/6 appointment types . Screening questions; confirmation/follow-up SMS/email; overflow logic. Analytics dashboard (containment, bookings, AHT, latency, drop-off reasons). Spanish voice model; custom name per practice; fallback to live receptionist. Day 45 (Launch tier parity + ortho workflows): Up to 6 providers / 10 appointment types ; ortho-specific intents (consults, banding/debanding, wire poke/repair, Invisalign shipment/pickup, tray loss). Calendar conflict handling, provider constraints, blocked time, office closures. Rescheduling edge cases + cancellation policies; recall + no-show automations. HIPAA review complete; SLOs defined ( 80% containment on after-hours calls); load test & playbooks.Qualifications 3–5+ years buildingvoice/real-timeapps with Twilio (or similar) and LLM-driven assistants. Hands-on withRetell AI(or equivalent real-time LLM voice stack) and tool/function calling. DemonstrableEHR/PMSintegration experience (ideally dental/medical) or strong API integration background; NexHealth experience is a plus. Solid backend engineering (Node/TS or Python), relational schemas, webhooks, queues,IaC on AWS , and production monitoring. Strongprompt engineeringfor voice UX: barge-in, repair strategies, memory scoping, guardrails. Security mindset; shipped somethingHIPAA-aware(or similarly regulated).Nice to Have Open Dental/Dentrix/Cloud 9/Dolphin ecosystem familiarity. Multilingual TTS/STT tuning; phonetic dictionaries; latency profiling.
