We foster a culture of collaboration, respect, and continuous learning.Job ResponsibilitiesPatient Admission & AssessmentPerform rapid tests immediately after patient admission using proper PPE. If the result is negative, conduct a detailed physical examination and document the findings.Communicate with the patient or relatives during admission to understand chief complaints, previous treatments, medications, medical and surgical history, and drug allergies. All information must be documented in the IPD case sheet.Consultations & CoordinationPlan and coordinate all consultations (online/offline) with external consultants and ensure timely follow-up as advised.Share and discuss all updates after MDT rounds with supervisors and the shift in-charge.Patient Care & MonitoringCross-check medication charts to ensure compliance with medication management protocols.Assist consultants in procedures such as ECT, EEG, etc.Document pre-procedure checklists for ECT.Conduct regular bedside visits to monitor patient condition, especially after administration of high-risk medications.Conduct ward rounds every 2 hours to ensure patient safety and maintain ward discipline.Manually check BP of all patients at 7:30 AM and 4:30 PM to verify the accuracy of vitals recorded by nursing staff.Hospital Policies & QualityAdhere to the hospital’s Infection Control Policy, including respiratory hygiene and cough etiquette.Participate in Continuous Quality Improvement Programs.Inform patient families about any procedures or investigations along with confirmed charges prior to conducting them.Attend departmental meetings and training sessions whenever required.Documentation ResponsibilitiesComplete basic documentation in the case history sheet within 24 hours, with full completion within 48 hours of admission.Record non-drug orders, investigation orders, and fluid orders in the relevant columns of the drug chart.Document patient assessments in progress notes during every shift.Print and attach prescriptions from online or teleconsultations in the patient’s medical record.Prepare an injury report immediately after admission and inform the patient’s relatives.Ensure proper consent forms for procedures such as ECT are obtained from the patient’s family or next of kin and attached to the patient’s file.Update drug orders immediately after internal or external consultations to avoid delays in medication administration.Prepare the draft discharge summary, including prescribed medications, before the discharge day.Update the patient’s diagnosis and medications in Practo at the time of discharge.Update patient condition, admission details, and any significant changes or external consultation details in the official WhatsApp group “Updates – IP Ward.”Provide a detailed written shift handover in the handover register, signed by both “Handover Given By” and “Handover Received By.”
