Summary With the growing challenge of non-communicable diseases (NCDs) such as diabetes and hypertension, Americares India Foundation is implementing the NC-DISHAA (Non-Communicable Disease Interventions for Screening, Health Awareness and Access) program in Mumbai. The initiative follows a four-pillar model: Establish, Educate, Engage, and Encourage.In its first phase (2019–2023), the program screened over 272,000 adults across 118 communities in 6 administrative wards of Mumbai and identified more than 24,000 individuals with diabetes and 26,000 with hypertension. Follow-ups were conducted quarterly by trained community health volunteers, known as Aarogya Mitras. The second phase (2023–2027) is currently underway, reaching over 15,000 patients across 86 locations in 5 administrative wards. It integrates one-on-one and group counselling, monitoring clinical parameters like HbA1c and LDL-C, and enhancing referral systems to ensure sustained care.Programme Associate is overall lead and responsible for the implementation of non-communicable disease intervention project in Mumbai. He/she will also be responsible for collection and compilation of data and reports. Candidate is also expected to take part in program planning and development. Further he / she will carry out any such work assigned to him/her as per the objects of the organization for the benefit of the underprivileged, destitute, victims of calamities and disasters, which include humanitarian relief & rehabilitation, provision of medical relief and aid during disaster, health care programs for strengthening services in education, health, and human welfare. Job Description of the PositionMain areas of responsibilities will include but not be limited to: Support in identifying, selecting and recruiting community health volunteers (Arogya Mitra: AM), project coordinators and Community Health Assistants for the projectDevelop micro plan for the project based on agreed up deliverables to the donor.Handholding of the project team members for adherence, execution and quality implementation of the project activities.Guiding and providing support to coordinators and community health assistants in their day-to-day operationsDevelop wok plan with targets for each Coordinators & community health assistant. Ensuring training of project staff in coordination with the medical team for community screening, testing, BCC sessions, community awareness programs, data collection, etc.Ensure timely capacity building of private and public health staff as per the workplan.Liasoning with government organizations and secondary/ tertiary health facilities – seeking permissions, sharing updates, conducting follow upsConduct field visits to monitor quality of services Independently conduct basic analysis of achievements and guiding the team accordingly In coordination with MEL team, collate, report and document monthly progress of the program to be shared in monthly review meetingsWork with program manager for daily reporting of dataAnalyze site wise data and provide inputs to Arogya Mitras (Community Health Volunteers), Community Health Assistants and coordinators for improving achievementsTrack systematically referrals of identified patients to public health facilities and updating team lead on the progress.Conduct periodic review meeting with the team and represent in the required forums.Identify and develop relationships with community stakeholders for implementation of community engagement activities.Ensure attendance, payments, voucher maintenance etc. is done as per procedures with admin and finance.
