We work on a clinician-led triangle model — a doctor, a functional fitness trainer, and a dietitian working in tandem around every patient. Our programmes target the root causes of Type 2 Diabetes, Hypertension, and Obesity through structured, evidence-based protocols — not just symptom management.We run a structured reversal programme through our proprietary fitness platform where patients log workouts, track progress, and stay accountable. The fitness trainer is not a peripheral role here — they are one of the three clinical pillars of the programme.ROLE OVERVIEWWe are looking for a qualified Physiotherapist who also knows the gym floor. A BPT or MPT degree is the non-negotiable foundation for this role — the clinical understanding that comes with a physio background is essential for working safely and effectively with patients who have diabetes, hypertension, or obesity. On top of that, you need to have genuine interest, knowledge, and hands-on experience as a fitness trainer — someone who is equally comfortable designing a resistance programme as they are reading a patient's clinical notes.You will be the primary point of contact for new patient onboarding on the fitness side, conduct baseline and periodic assessments, design individualised functional fitness plans, and run structured training sessions as part of the reversal programme. You will work alongside a doctor and a dietitian on every case — the physio background makes you the most clinically equipped person in that triad to understand movement, pain, compensation patterns, and exercise safety.KEY RESPONSIBILITY AREAS (KRAS)New Patient OnboardingConduct the initial fitness onboarding session for every new patient enrolled in the reversal programmeComplete a structured baseline assessment: movement screen, functional fitness level, mobility, strength, and cardiovascular enduranceReview patient history, doctor notes, and dietitian inputs before onboardingBrief patients on the fitness module of the reversal programme — what to expect, how progress will be measured, and how to use the fitness app2Assessment & PlanningDesign a personalised functional fitness plan based on baseline assessment, medical condition, current fitness level, and programme goalsPlans must be specific to the metabolic condition — accounting for blood glucose variability in diabetics, safe exertion limits for hypertensives, and progressive overload protocols for obese patientsAlign the fitness plan with the doctor's clinical targets and the dietitian's nutrition scheduleDocument all assessments and plans on the ReDiaL fitness app platform3Training SessionsConduct regular 1:1 or small-group training sessions with patients as part of their structured reversal planDeliver functional movement, resistance, and cardio-metabolic training protocols suited to each patient's condition and stageEnsure proper form, safe progression, and appropriate modifications for metabolic patientsLog every session on the fitness app with notes on performance, effort, and modifications4Periodic Reviews & Plan RevisionsConduct structured re-assessments at defined intervals (typically 4 and 8 weeks) to track progress against baselineRevise fitness plans based on clinical progress, patient feedback, and doctor reviewIdentify patients who are not progressing and flag to the clinical team with observationsMaintain updated records on the ReDiaL fitness app post every review5Patient Engagement & AccountabilityMonitor patient adherence to their fitness plans via the app and follow up where neededMotivate and educate patients on the role of exercise in metabolic disease reversalRespond to patient queries related to their fitness plan in a timely and clinically responsible mannerCoordinate with the doctor and dietitian to ensure consistent patient communication6Clinical CoordinationParticipate in weekly inter-disciplinary reviews with the doctor and dietitian teamRaise flags on exercise-related clinical concerns (e.g., dizziness, fatigue patterns, blood pressure response during exercise)Adapt protocols based on medication changes or dietary shifts recommended by the clinical teamMaintain complete, updated patient records on all designated platformsQUALIFICATIONS & CERTIFICATIONSREQUIREDBPT (Bachelor of Physiotherapy) or MPT (Master of Physiotherapy) — mandatory qualification, non-negotiableCertified Personal Trainer (CPT) — ACE, NASM, ACSM, ISSA, or equivalent recognised certificationHands-on gym training experience — must have worked with clients in a gym or fitness setting, not only in a clinical rehab environmentMinimum 2 years of combined physiotherapy and fitness training experienceDemonstrated experience or strong interest in working with patients with metabolic disorders: Type 2 Diabetes, Hypertension, or ObesityComfort working within a clinical, multi-disciplinary team environmentAbility to use digital tools, apps, and maintain patient records accuratelyPREFERRED (STRONG ADVANTAGE)MPT specialisation in Musculoskeletal, Sports, or Cardiopulmonary PhysiotherapyPrior experience in a clinical fitness, rehab gym, or medically supervised exercise settingSpecialised certification in Diabetes Exercise Management, Cardiac Rehab, or Obesity & ExerciseSports background — competitive athlete or sports performance coaching experience alongside physioFamiliarity with wearables, CGM interpretation in an exercise context, or HRV-based trainingExperience with clinic-integrated or medically supervised fitness programmesWHAT GOOD LOOKS LIKE IN THIS ROLEThe best person for this role understands that training a 52-year-old Type 2 diabetic is fundamentally different from training a 28-year-old who wants to lose weight.
