This will include those with chronic illnesses, post hospitalisation care, wound management, and palliative care (this list is not exhaustive).The role involves collaborating with other healthcare professionals and the wider MDT, supporting patient recovery, and promoting self-care in negotiation with patients, their carers and families.Main duties of the jobAssess, plan implement, and evaluate patient care in community settings.Provide wound management including dressings, compression therapy, and other related interventions.Administer medications, including injections.Support patients with chronic disease management (e.g. diabetes, respiratory conditions).Deliver End-of-Life care and support patients with palliative care needs.Health education and self-care facilitation for patients to promote their independence.Conduct initial assessments and regular reviews of patients health conditions.Maintain accurate and up-to-date clinical records.Ensure adherence to infection control and health and safety protocols.Monitor patients for any changes in their condition, responding appropriately to emergencies and changes.Refer patients to other healthcare professionals when necessary.Work closely with GPs, physiotherapists, occupational therapists, social workers and other healthcare professionals to ensure holistic care.Provide support and advice to patients families or carers, ensuring they are involved in the care process.Attend Multidisciplinary team meetings and contribute to care planning.Engage in continuous professional development (CPD) to keep update with the latest nursing practices and guidelines.Supervise and mentor junior nurses or nursing students when required.Participate in audits, quality improvement projects and research to improve community healthcare delivery.About UsWe are a provider of NHS Community Health Services, CityCare exists to support the health and wellbeing of all local people, working alongside other health and care partners to achieve this.
