What you will do: Conduct statutory Child and Family assessments in accordance with CA 89 and the Chronically Sick and Disabled Act. Develop and chair the implementation of Child in Need and Child Protection plans as required. Engage effectively with children and young people facing communication challenges. Evaluate risks to children with disabilities and propose sound protection measures. Identify suitable short break services for children with disabilities. Manage Children in Care, ensuring care plans are comprehensive and current. Conduct court work when necessary, ensuring the best outcomes for the children and their families. Facilitate the transition for young adults into adult services, crafting Transition Plans that meet their needs.
