Grade 8 Social Worker South CMHTThe Mental Health South Team has a vacancy for a permanent Grade 8 Social Worker to join the Mental Health South Social Work team based at Delamere Resource Centre. We are currently adopting a hybrid working model, combining office and home working. The successful applicant will report to and be supported by Practice Manager Candice Holt. The Mental Health Social Work Team, based within the Community Mental Health Team (CMHT) at Delamere Resource Centre in Crewe, provides assessment and support to individuals aged 18 and over experiencing mental health difficulties that impact their ability to function fully in daily life. The successful candidate will be expected to manage a caseload and work in accordance with statutory responsibilities under the Care Act and Mental Capacity Act, including completing assessments and support plans for supported placements and care at home. You will work within a multidisciplinary team supporting joint working with individuals, families, and carers. We are seeking an individual who can demonstrate the ability to promote the rights, dignity, and self-determination of individuals with a wide range of mental health issues and vulnerabilities, and who has a passion to develop further skills in mental health.
