This position is available in the Chalfont's and Gerrards Cross area to join a newly formed team as a Community Nurse.We welcome highly motivated, effective team players committed to providing high-quality nursing services within the District Nursing field. As a community nurse, you will be supported to become an autonomous practitioner, caring for patients with complex healthcare needs in their homes.Community Nursing Practice Community nursing is a specialised practice area. As a Community Staff Nurse, you will work within a locality team comprising District Nurse Team Leaders, Community Staff Nurses, Therapists, Assistant Practitioners, and Healthcare Assistants. Your role will involve caring for patients with complex needs in their homes and residential settings, including:Medication administrationEnd of life careManaging deteriorating patientsWound care, including negative pressure and compression therapyVenepunctureUtilising advanced assessment and clinical skillsYou will work closely with social care, GPs, community therapies, and specialised services such as Tissue Viability, Diabetes, and Heart Failure.Why Join Us?
